The real hazard is if the x-rays affect the DNA of cells.

D N A . . . that's like the brain of a cell.

That's the one, good ol' DeoxyriboNucleic Acid. The DNA tells the cell how to reproduce, or make a copy, of itself. If the DNA is damaged, this damage can be passed on to new cells. For the levels of radiation you get at the dentist and the doctor, the body is able to repair most all of the damage done.


If that's the case, why don't those dentists and doctors stay in the room with you? They must know something that I don't know.

Having x-rays taken a couple times a year isn't a big deal. But why should dentists and doctors be exposed to x-rays day in and day out as a part of their job? There is no need for them to take that risk.

All of this stuff with x-rays has something to do with cancer, right?

That's right. Cancer is a group of cells with damaged DNA that are out of control and making too many copies of themselves. Keep in mind that there are many steps to go from DNA damage to cancer.

After all you've said, is it really safe to have x-rays taken at dentists and doctors offices? I don't want to get cancer.

Low doses of x-rays cause a negligible increase in the risk of cancer. Don't forget, there are many things that can cause cancer, from various chemicals to radon gas and cigarettes. X-rays are a mixed blessing. If no x-rays are absorbed by the body, the image would come out white - that's not useful to a doctor. So some x-rays have to be absorbed. That's why dentists and doctors give low doses of x-rays to you, enough to see what is going on inside of you while minimizing any risk to you.

Darn, I was hoping for a scientific excuse for avoiding the dentist.

Not this time.

So why are x-rays bad?

We just saw that x-rays go into matter but not all of them come out. What happens to the missing x-rays?

So they check in but they don't check out? Sounds like a roach motel.

X-rays aren't alive like roaches, but something happens to them. The missing x-rays get absorbed in the body, disappear if you like, and the x-ray's energy is released.

Can that energy harm the human body?

Yes, Kyla. There is a lot of energy in a single x-ray, and the energy is transferred to an electron, which does a lot of damage in a small area. It's a lot like playing a game of pool.

Pool? Not another wacky analogy.

The absorbed x-ray gives all of its energy to an electron, and this electron can rip through a cell, tearing electrons from all sorts of atoms and molecules it meets. It is like a billiard ball spreading its energy to all of the other balls on the table. The fancy name for this process is ionization, because taking away electrons makes the atoms and molecules into ions.

But that doesn't seem very bad at all.

These high speed electrons do damage to the cells. Fortunately, the human body is good at repairing itself.

Why is tungsten used in x-ray tubes? Can't other elements produce x-rays?

Most elements emit x-rays when properly bombarded with electrons. Heavier elements (like tungsten) are best because they emit a higher intensity through bremsstrahlung, but there are plenty of heavy elements to choose from. The real issue is engineering: Most electrons that hit the tungsten don't do anything special at all -- no bremsstrahlung, no K-shell emission. All of the energy from the electrons' impact then goes into heating the tungsten. Tungsten is used because it can withstand this bombardment, as it has a high melting point and can conduct heat away very well.

What would happen if you replaced the tungsten with something else?

The bremsstrahlung pattern looks very similar no matter what element you use. The K-shell emission spectrum is unique and different for each element.

In bremsstrahlung, why is a range of photons emitted instead of just one wavelength?

The incoming electron is accelerated and strikes the tungsten at a high speed and has a lot of energy. Recall that we called it "braking radiation." The electron might be slowed a little or a lot.

So the amount of "braking" determines which wavelength of photons are emitted.

Yes, but there is more. If we represent all of the energy of the electron as a pie, there are bazillions of different ways of cutting up this pie.


 

But one thing's for sure, you can never end up with more pie than you started with. If all of the energy goes into producing only one photon, there is no way you could have a photon with more energy than that!

So there should be a sharp cut-off in the spectrum. It looked like there was a cut-off on each end.

No, there is only one cut-off that corresponds to a minimum wavelength. There is no limit to a maximum wavelength emitted. Go back to the bremsstrahlung spectrum and see how it fades gradually to zero for long wavelengths.

Where do x-rays come from?

An x-ray machine, like that used in a doctor's or a dentist's office, is really very simple. Inside the machine is an x-ray tube. An electron gun inside the tube shoots high energy electrons at a target made of heavy atoms, such as tungsten. X-rays come out because of atomic processes induced by the energetic electrons shot at the target.

X-rays are just like any other kind of electromagnetic radiation. They can be produced in parcels of energy called photons, just like light. There are two different atomic processes that can produce x-ray photons. One is called Bremsstrahlung, which is a fancy German name meaning "braking radiation." The other is called K-shell emission. They can both occur in heavy atoms like tungsten.

So do both ways of making x-rays involve a change in the state of electrons?

That's right. But Bremsstrahlung is easier to understand using the classical idea that radiation is emitted when the velocity of the electron shot at the tungsten changes. This electron slows down after swinging around the nucleus of a tungsten atom and loses energy by radiating x-rays. In the quantum picture, a lot of photons of different wavelengths are produced, but none of the photons has more energy than the electron had to begin with. After emitting the spectrum of x-ray radiation the original electron is slowed down or stopped.

What is the "K-shell" in the other way of making x-rays?

Do you remember that atoms have their electrons arranged in closed "shells" of different energies? Well, the K-shell is the lowest energy state of an atom.

What can the incoming electron from the electron gun do to a K-shell electron in a tungsten target atom?

It can give it enough energy to knock it out of its energy state. Then, a tungsten electron of higher energy (from an outer shell) can fall into the K-shell. The energy lost by the falling electron shows up in an emitted x-ray photon. Meanwhile, higher energy electrons fall into the vacated energy state in the outer shell, and so on. K-shell emission produces higher-intensity x-rays than Bremsstrahlung, and the x-ray photon comes out at a single wavelength. Have a look at both mechanisms in the experiment below.

اثر بر محیط کار

در تماس با اسیدها بخارات سمی نظیر دی اکسید گوگرد و سولفید هیدروژن تولید میکند. در تماس با داروی ظهورdevalex   گاز آمونیاک آزاد میکند.

 

 

اثر بر محیط زیست

در کوتاه مدت موجب کاهش اکسیژن محلول در آب شده و اکوسیستم آبی را تخریب میکند.

 

4- احتیاطهای ایمنی – بهداشتی:

تماس با چشم

به سرعت و با استفاده از چشم شوی یا پیست محتوی آب تمیز چشمها را بشوئید و بلافاصله به پزشک مراجعه نمائید.

تماس با پوست

بلافاصله همه لباسهای آلوده به این ماده را ازتن خارج کنید و پوست را با آب سرد و یک پاک کننده طبیعی و غیر قلیائی ( غیرصابونی) شست و شو دهید . درصورت مشاهده هرگونه تحریک پوستی به پزشک مراجعه کنید.

بلعیدن و خوردن

دهان را با مقادیر زیادی آب پاکیزه بشوئید بعد حدود 2 لیوان آب بنوشید وادار به استفراغ نکنید و بسرعت به پزشک مراجعه نمائید.

تنفس

فرد مسموم را به هوای آزاد منتقل کنید . در صورت بروز مشکلات تنفسی به پزشک مراجعه نمائید.

حریق

قابل اشتعال نیست – در صورت قرارگرفتن در مجاورت آتش گازهای سمی H2S  , SO2  متصاعد میکند

انفجار

خطر انفجار ندارد.

 

 

5- احتیاط در زمان وقوع حادثه:

حفاظت فردی

در هنگام ریختن  محلولهای  با حجم زیاد در ظروف کوچکتر ویا رقیق سازی محلولهای غلیظ از لوازم حفاظت فردی به شرح بند 7 این دستورالعمل استفاده نمائید . ضمن اینکه باید از وجود سیستم تهویه مناسب در محل کار مطمئن باشد.

حفاظت محیطی

هرگز این ماده را در حجم زیاد داخل  راه آب فاضلابهایی که بدون تصفیه به چاه جذبی دارای مواد جاذبی نظیر ماسه خشک ویا زمین منتهی میشوند تخلیه نکنید. محلولهای اضافی را برای دفع در محفظه مخصوص دفع  موادشیمیایی ریخته و به افراد باصلاحیت بمنظور دفع بهداشتی تحویل دهید.

نظافت

بعد از جابجائی  محلول در ظروف کوچکتر و چیدمان آنها محل را کاملا از مایعات ریخته شده پاک نموده و با آب بشوئید.

 

6- جابجائی ( دستی)و انبارداری :

جابجائی ( دستی)

در هنگام جابجائی محلولهای غلیظ مراقب چشم- پوست و لباس خود باشید که آلوده نشود. از وجود سیستم تهویه مناسب در محیط مطئن باشید.

انبارداری

در محل خشک و دارای تهویه مناسب و دمای متوسط (معمولی) نگهداری کنید. از نگهداری در مجاورت ترکیبات ناسازگار جلوگیری کنید( به بند 8 این دستورالعمل مراجعه نمائید)

                                                             

7-حفاظت فردی:

محدودیت تماس

ممکن است در هنگام آماده سازی محلول تماس با محلولهای غلیظ و همچنین بخارات اسید استیک و دی اکسید گوگرد رخ دهد که میبایست مدت زمان مواجهه براساس جداول استاندارد TLV ( وزارت بهداشت) باشد[1]

حفاظت مهندسی

تهویه عمومی و موضعی قوی نیاز میباشد.

حفاظت از دستها

از دستکش محافظ مخصوص مواد شیمیایی استفاده کنید. دستکش لاتکس نامناسب است .از تماس پوستی با این ماده اجتناب کرده و برای شست و شوی دستها از محلولهای غیر صابونی استفاده نمائید .

حفاظت از چشم ها

از عینک محافظ با شیلدکناری استفاده کنید( حداقل میزان حفاظت بعمل میآید). چشم شوی و یا پیست محتوی آب تمیز برای شست و شوی چشمها باید دردسترس باشد.

حفاظت از سیستم تنفسی

درصورت کافی بودن سیستم تهویه نیاز به استفاده از ماسک  شیمیائی نیست.

 

8-اطلاعات سم شناسی:

تماس با چشم

در تماسهای حاد موجب تحریک و سرخی چشم میشود. آسیب شدیدی بجز تحریک و التهاب چشم ایجاد نمیکند.

تماس با پوست

باعث تحریک پوست میشود.

بلعیدن

باعث حالت تهوع و استفراغ میشود ولی ایجاد مسمومیت سیستمیک نمیکند.

باعث تحریک دهان و گلو شده ولی آسیب شدید ایجاد نمیکند. در تماسهای مزمن و طولانی مدت باعث تجمع آلومینیوم در بدن میگردد( بدلیل وجود سولفات آلومینیوم)

تنفس

احتمال تحریک یا سوختگی پوشش مخاطی سیستم تنفسی وجود دارد .در شرایط عادی هیچگونه بخارات سمی از این ماده متصاعد نمیشود.

موجب تحریک سیستم تنفسی – آسم و سایر مشکلات تنفسی میشود.

برای سلامتی انسان مضر بوده و قادر به ایجاد تحریکات پوستی و مخاط چشم میباشد حتی ممکن است باعث حساسیت پوستی شود. اطلاعات کافی  در زمینه سرطانزائی این ماده در دست نمیباشد.

در تماس با اسیدها بخارات سمی نظیر دی اکسید گوگرد تولید میکند. در تماس با داروی ثبوتFixaplus  گاز آمونیاک آزاد میکند.

اثر بر محیط زیست

در کوتاه مدت موجب کاهش اکسیژن محلول در آب شده و قلیائیت آب را افزایش میدهد و موجب از بین رفتن گیاهان و موجودات زنده میشود.

 

4- احتیاطهای ایمنی - بهداشتی:

تماس با چشم

به سرعت و با استفاده از چشم شوی یا پیست محتوی آب تمیز چشمها را بشوئید و بلافاصله به پزشک مراجعه نمائید.

انبارداری

در محل خشک و دارای تهویه مناسب و دمای متوسط (معمولی) نگهداری کنید. از نگهداری در مجاورت ترکیبات ناسازگار جلوگیری کنید( به بند 8 این دستورالعمل مراجعه نمائید)

تکرارتماسهای پوستی در دراز مدت باعث بروز تحریک و حساسیت پوستی میشود.

بلعیدن

بلعیدن محلول غلیظ باعث تحریک دهان و معده میشود . مسمومیت سیستمیک بعلت بلع این ماده انتظار نمیرود.

تنفس

احتمال تحریک یا سوختگی پوشش مخاطی سیستم تنفسی وجود دارد .در شرایط عادی هیچگونه بخارات سمی از این ماده متصاعد نمیشود.

The common elements of technique will be present below.

Accessories:
Fine and standard/regular resolution, film speed screen combinations in an assortment of sizes.
Pads and immobilisation aids.
Cones and Plates to fit equipment being used.
Hard surface disinfectant for bucky board or head support.

Anatomical Markers
Anatomical markers placed on the cassette at the time of exposure with the correct PA AP orientation is the preferred standard in general use in the UK. Additional special radio opaque markers may be used in special circumstances. Anatomical markers should be placed in the primary beam away from any relevant anatomical structure and the correct way up / round.

Collimation
Collimation to include the required structures within the size of the film required is generally the best description, reducing the volume of tissue irradiated reduces the dose to the patient.
Evidence of collimation on four sides equally around the centering point 
However collimation may sometimes be compromised by the need to center over a specific point such as a joint space, and include for example the lower third of the tibia and fibula in imaging the ankle, in this case the distal collimation may not be visible and  may be off the film.
Collimation in general should be such that the radiosensitive organs such as the thyroid, eyes, gonads and breasts receive the least dose commensurate with the image required.

Immobilisation
Full immobilisation is essential for high quality diagnostic imaging, the use of supporting devices, Velcro binders and pads will assist in immobilising the patient in most cases.

Indication for Imaging

FFD Focus to Film Distance
In general radiographic imaging is performed at two common FFD's
100cm and 180cm, in general the 100cm is used for non bucky work where there is little magnification, and 180cm when there is increased risk of magnification, however in bucky work must be performed at the required FFD for the grid involved, this is most commonly between 100 and 130 cm with some specialised skull units using values less than 100cm.

Before starting any examination, the identity of the patient must be checked by the radiographer; a patient may answer to a name not his/her own. If the patient is fully compus mentus, the radiographer should ask the patient their name and date of birth, these must match the request form, Inpatients must have their ID bands checked and they must match the request card.

Patient Preparation
Basic psychological preparation with reassurance and explanation of technique.
Patients referred for radiography may be worried and anxious about the outcome, some patients, are difficult to handle and may need special care, typically the very young, old  physically and or mentally infirm, unconscious or unable to co-operate, The assistance of a nurse or other competent person may be required. An understanding and tolerant attitude exhibited by the radiographer always helps. Time taken to explain the test to the patient is never wasted.
All detachable artifacts must be removed from the image area, long hair must be tied out of the way, if possible monitoring leads /pads should be removed for the duration of the examination.
It is important to remember the dignity of the patient, and essential to have clean hands, a clean cassette or bucky stands and clean immobilisation aids at all times.

Radiation Protection
The 10 or 28 day rule will apply as required (see SP8 in the regulations section)
Methods for reducing doses to patients should be followed at all times.
Where possible beam should be pointed away from the gonads or most radiosensitive organs, there should be direct lead rubber gonad protection whenever possible taking into account the possibility of back scatter from equipment. The most effective method of dose reduction is careful technique to avoid the need for repeat radiographs.

Angiography Test

What is angiography؟

Angiography is a way to produce X-ray pictures of the inside of blood vessels. When blood vessels are blocked, damaged or abnormal in any way, chest pain, heart attack, stroke or other problems may occur. Angiography helps your physician determine the source of the problem and the extent of damage to the blood vessel segments that are being examined.

Before the test

Lab work may be needed before the angiogram to determine your blood's ability to clot.

Please follow these guidelines after midnight the night before your test:

Medications

Always consult with your primary physician and/or physician requesting this test before discontinuing any medication. Here are some guidelines:

  • Do NOT take any aspirin or any products containing aspirin.
  • Do NOT take dipyridamole (Persantine) or warfarin (Coumadin) within 72 hours before the test, and 24 hours after the test. These medications are often referred to as blood thinning pills.
  • DO take your other medications on schedule as usual, especially any medications for high blood pressure.

If you have diabetes

  • Take half of your usual dose of insulin and eat a light breakfast before 6 a.m. When you arrive for your test, please be sure to remind the physician that you have diabetes and have eaten breakfast.
  • Do NOT take Glucophage (metformin hydrochloride) for 48 hours before the test or 48 hours after the test to reduce the risk of kidney complications.
Eating and drinking
  • Drink only clear liquids for breakfast the day of your test. Clear liquids include clear broth, tea, strained fruit juices, strained vegetable soup, black coffee, plain gelatin, tomato juice and ginger ale.

On the day of the test

Please make arrangements for transportation, as you should not drive until the day after the procedure.

Please do not bring valuables such as jewelry or credit cards.

You will meet with your physician and he or she will review further instructions and discuss any questions you may have. The physician will also review your medical history.

In almost all cases, laboratory tests ("blood work") will be needed, and your physician will need to study the results before beginning your angiography test. It may be several hours before the laboratory results are available.

Every effort will be made to perform your test at the scheduled time, but delays do occur. You may want to bring a book or magazine.

During the test

You may be asked to change into a hospital gown.

The test itself will take approximately 2 to 3 hours. Mild sedation may be given.

During angiography, a long slender tube called a catheter is inserted into a large artery (generally, in the groin area).

The catheter is slowly and carefully threaded through the artery until its tip reaches the segment of vessel to be examined by angiography.

A small amount of contrast material is injected into the blood vessel segment through the catheter, and X-rays are taken. The contrast agent enables the blood vessels to appear on the X-ray pictures.

A physician specially trained in angiography studies the X-ray pictures to determine the source of the problem and the extent of damage to the blood vessel segments that are examined.

After the test

You will be monitored for 4 to 6 hours. At that time, the radiology nurse will discuss post-angiogram instructions with you. You will be provided with a written form of these instructions-- please follow these at home.

A radiologist will evaluate you before you are discharged. Then your physician will discuss the test results with you.

If you have diabetes, do NOT take Glucophage (metformin hydrochloride) for 48 hours after the test to reduce the risk of kidney complications.

The evening after the test

We advise for your safety to spend a quiet, restful evening with a friend or relative in the immediate area. On rare occasions, it may be necessary for you to spend the night in the hospital.

Angiography allows your physician to view how blood circulates within vessels at specific locations in the body. This diagnostic test is used to locate the specific source of an abnormality in the neck, kidneys, legs or other sites.

آنژیوگرافی

به تناسب کاربردهای مورد نظر ( قلب،‌ سر و یا اندام شکمی )و بودجه ای که اختصاص می یابد ،‌ سیستم های متفاوتی را می توان پیشنهاد نمود .

سیستم آنژیوگرافی عروق کرونر و کاردیوآنژیوگرافیدر سیستم های آنژیوگرافی  سیستم  گردش خون عموماً از تکنیک سریال رادیوگرافی سریع با کمک فیلم  چنجر  استفاده   می شود  . با  به   بازار آمدن تیوبهای مولد اشعه میکروفوکوس با ظرفیت حرارتی بسیار بالا ، لامپهای تشدید کننده تصویر فلزی با کیفیت بسیار بالا و سایر تجهیزات از جمله ریزپردازنده های  بسیار  سریع  ،  امکان  معاینات دینامیک سیستم گردش خون  توسط  دوربین های 25 میلی متری ( سینه کامرا ) با سرعتهای بسیار بالا ( 100 کادر در ثانیه ) امری بسیار معمول تلقی می گردد .در تکنیک سینه فلوروگرافی عواملی مانند فلوروگرافی دوجهتی( bi-plane ) کیفیت ارتقاء یافته ابزار تصویربرداری ، امکان کار سریع ،‌ پرکیفیت و آسان ،  multiprojection fluorography ، موجب محبوبیت سیستم و ارتقاء قابل ملاحظه نتایج کار گردیده است . در تکنیک multiprojection نه تنها چرخش سیستم تصویری حول بدن بیمار در محور طولی بلکه در وضعیت های کرانیوکودال و یا کودوکرانیال نیز امکان پذیر گردیده است .برای دستیابی به کیفیت مطلوب تر تصاویر عوامل زیر نقش بسیار اساسی دارند :·         ظرفیت حرارتی بالاتر تیوبهای مولد اشعه با فوکوسهای بسیار کوچک              “Ultra fine focus”
·         لامپ تشدید کننده تصویر با قدرت تفکیک بسیار بالا·         رویه تخت با ضریب جذب بسیار پائین·         دوربین های بسیار دقیق·         مانیتورهای با قدرت تفکیک بسیار بالا·         انتخاب صحیح نوع فیلم·         سیستم ظهور و ثبوت متناسب با فیلم مورد استفاده·     مانور زاویه ای و چرخشی سیستم تصویری ( در انطباق با تیوب مولد اشعه ) در تمام حالات و زوایای
ممکن .
سیستم ایزوسنتریک انطباق دادن منطقه مورد نظر ( ROI ) با سیستم تصویری را تضمین می کند .مجهز بودن سیستم به بازویARM- C فضای باز مورد نیاز در اطراف سر و مواضع شکمی بیمار را تأمین نموده ، بدون اینکه زاویه گرفتن تخت را در وضعیت Caudo-cranial محدود سازد .
سیستم آنژیوگرافی محیطی ( سر و شکم و ... )
در آنژیوگرافی محیطی ( سر و شکم ) مواضعی مورد معاینه قرار می گیرند که در آنها عروق خونی بسیار قرار گرفته اند و طبیعتاً عروق باریکتر توسط عروق عریضتر پوشانده شده و مشاهده آنها را مشکلتر می نماید . تکنیک بزرگنمایی استریوسکوپیک توانسته است به خوبی این مشکل را آسان نماید .این تکنیک امتیازات چندی دارد که در ذیل به برخی اشاره شده است :تصاویر با بزرگنمایی مطلوب ،‌ عروق بسیار باریک و مواضع بسیار کوچک آسیب دیده را به راحتی از هم تفکیک نموده و عروق بسیار نزدیک به یکدیگر را قابل مشاهده و بررسی می نماید .این تکنیک به خوبی میتواند بدون تأثیر از پرتوهای پراکنده تصاویری عاری از تداخل و اعوجاج با کنتراست بالا ایجاد نماید .ارتقاء کیفیت تشخیص در مواردی که آنژیوگرافی روتین ( معمولی ) پاسخگو نمی باشد ، با تکنیک استریوسکوپیک عملی است .در آنژیوگرافی محیطی فاکتور عمده در انجام کار سریع و دقیق ، عدم نیاز به تغییر وضعیت بیمار است ، زمانی که سیستم از حالت فلوروسکوپی به رادیوگرافی تغییر حالت می دهد . بنابراین تختی که رویه آن بتواند با مانور حرکت طولی کافی بیمار را بدون نیاز به تغییر وضعیت در میدان تابش رادیوگرافی قرار دهد از ابزار اصلی و پایه ای به حساب می آید .برای آنژیوگرافی اندام تحتانی بدن نیز تختی که رویه آن با گام های حرکتی قابل تنظیم حرکت نماید و از این طریق امکان اکسپوزهای سریال را فراهم نماید ، بسیار حائز اهمیت است .در عصری که سیستم های توموگرافی کامپیوتری ( CT)مطرح اند ،‌ سیستم های آنژیوگرافی که بتوانند با افزایش توانمندی های تشخیصی در عین حال بتوانند ریسک مشکلات جانبی برای بیمار را کاهش داده و با دوز کاهش یافته پرتو X و مواد حاجب قادر به کار باشند ، هنوز می توانند جایگاه اختصاصی و ویژه خود را حفظ نمایند .در زیر دو نمونه از سیستمها معرفی میشوند:1.یک سیستم مجهز به بازوی نگهدارنده زمینی سی آرم و سی آرم نیمرخ برای آنژیوگرافی عروق کرونر و آنژیوگرافی قلبی (دو جهتی) .این سیستم سینه فلوروگرافی دوجهتی و نیز آنژیوگرافی عروق کرونر و نیز بررسی نحوه عملکرد سیستم قلبی حتی در نوزادان را ممکن می سازد .

Magnetic Resonance Imaging (MRI)     for parents

What is Magnetic Resonance Imaging (MRI)?

Magnetic Resonance Imaging (MRI) is a diagnostic imaging procedure that uses a powerful magnet and radiofrequencies to produce images of the body. The computer used in MRI allows us to produce either 2 or 3 dimensional images of structures. It is useful to see tissues anywhere in the body that otherwise may only be seen through surgery.

The MRI machine looks like a small tunnel. The exam table moves through the opening of the tunnel.

The MRI machine makes images that are different than a regular x-ray. It makes cross-sectional images of the body, most often referred to as slices. Just imagine looking at a piece of bread from the middle of the loaf — thats what an MRI machine can do!

What should you do prior to the exam?

There are different preparations to follow according to exactly what type of MRI scan your child is having.

If your child is:

  • Not being sedated and is or is not receiving radiographic contrast, then there are no special preparations for your child to follow.
  • Being sedated for the exam, please follow the sedation preparation guidelines. Sedation is required for all children up to 8 years of age.
  • Having an MRI Cholangiogram, then your child may have nothing to eat or drink 4 hours prior to the exam time. If your child is also being sedated for this exam, please follow the sedation guidelines.

If your child has a noted allergy to radiographic contrast, additional preparations may be necessary.

Dress your child comfortably, in clothes that are easily removed (sweat clothes, t -shirts). Your child may be given a gown to change into for the MRI, if required.

If you have copies of your childs previous imaging studies from another institution, please bring them for comparison.

MRI Safety Precautions

The Magnetic Field used in the MRI process will also affect implanted patient devices and other various items. For this reason, every patient MUST be screened to ensure the child and parent or guardian are SAFE to enter the Magnetic Field.

ALL Implants or Devices must be approved by the MRI Technologists and Radiologists before entering the Magnetic Field. To ensure efficient screening, prior to arrival please obtain and bring with you the following information about your childs device:

  • Name of Manufacturer
  • Date of Placement
  • Name of Device and Composition (composition may not be listed but we can research it if needed)

The following implants/items ARE NOT allowed into the CHOP MRI:

  • Defibrillators (IAED)
  • Greenfield Filters
  • Pacemakers
  • Pregnancy (1st trimester)
  • Transdermal medication patches

The following implants/items MUST be evaluated (but not limited to the list below):

  • Bullets
  • Clamps
  • Clips
  • Cochlear Implants
  • Coils
  • Pins
  • Pregnancy
  • Screws
  • Shrapnel
  • Tattoos

The following implants/items require special care before and after entering the MRI Field:

  • IV Fluids
  • PCA pumps
  • Programmable VP Shunts
  • Vagal Nerve Stimulators

All jewelry and/or metal (zippers, snaps) must be removed prior to entering the scanner room.

Note: Only one parent/guardian will be allowed to accompany the child into the scanner room; other arrangements should be made for siblings. The parent/guardian accompanying the child to their MRI must be safety screened and changed appropriately, if needed, before entering the scanner room.

Parents/Guardians who are pregnant or may be pregnant will not be permitted to accompany their child during the MRI scan. Please make sure that there is someone else available to be with the child during the scan, if needed.

What should you do when you arrive?

If your appointment is scheduled at the Main Hospital (Wood Building):

You must check in to Wood Radiology Registration, on the 2nd Floor of the Wood Building, 1 hour prior to your scheduled appointment. This is where your registration process occurs.

The law requires that you bring a prescription from your doctors office stating the exam to be done and the reason for it.

If your insurance requires a referral, it must be presented at the time of registration. Proof of insurance will also need to be provided at this time.

Once the registration process is completed, the MRI Department will be notified. A technologist or nurse will come out to the central registration area and bring you and your child to a room for his or her pre-MRI work-up (screening, IV placement and/or sedation if needed).

If your appointment is scheduled at the Pediatric Imaging Center at King of Prussiar:

You must check in at the main registration desk, located in the main waiting room, 45 minutes prior to your scheduled appointment. This is where your registration process occurs.

The law requires that you bring a prescription from your doctors office stating the exam to be done and the reason for it.

If your insurance requires a referral, it must be presented at the time of registration. Proof of insurance will also need to be provided at this time.

Once the registration process is completed, you will be directed to the Radiology waiting room. The MRI technologist will be notified and a technologist or nurse will come out to the Radiology waiting room and bring you and your child to a room for his or her pre-MRI work-up (screening, IV placement and/or sedation if needed).

What should you and your child expect during the exam?


Magnetic Resonance Imaging (MRI)
If your child is receiving contrast and/or sedation, you will first be taken to a room for the pre-MRI work-up. To give the contrast and/or sedation, an IV will be placed in your child. The IV may be placed in his or her hand, arm or sometimes, foot. During the IV placement, your child may feel pain or discomfort. Once your child has proper IV access, and/or a sedation assessment is complete, you and your child will be taken to the MRI control room. In the MRI control room, the technologist will ask why the MRI is being performed, explain the scan to both you and your child, and will repeat the safety screening process.

You and your child will then enter the MRI scanning room with the technologist. The room may feel cold to you, so be prepared with layers as needed. You will be asked to take a seat in a chair located near the scanner. You may bring reading materials into the room to occupy yourself. The technologist will then help to position your child on the MRI table according to the type of scan he or she is having. While many children are positioned on their backs, some scans require children to lie on their stomach or side, head first or feet first. Due to the loud nature of the scan, you and your child will be given ear plugs to help block out some of the noise.

Once your child is comfortable on the table, a red light will come on to help align his/her body, ensuring the correct position for the scan. Your child will then be moved into the tunnel of the MRI scanner and, depending on his/her position, may not be able to see you or talk with you.

After the technologist leaves the scanning room, the MRI will begin. The MRI scanner makes many sequences of loud noises, each of which may sound somewhat different.

There will be a brief pause after each sequence. The technologist may give your child directions or prompts through a speaker. Your child will be able to communicate back to the technologist through the speaker.

The technologist will ask your child to hold very still during the scan. Any type of movement during the MRI will make the images appear blurry, so the scan may have to be repeated. This will lengthen the time the test will take.

Your child will not feel the magnetic field from the MRI scan, but sometimes holding still for a long period of time can be uncomfortable. The MRI scan can take from 30 minutes to 1 hour for each body part being examined. While some patients do not require sedation for their MRI, others may benefit from emotional and/or medicinal support. Remember, sedation is required for all children under the age of 8. Regardless of the needs your child may have, the MRI team is there to help him/her get through their experience in the best way possible.

Child Life Specialists are available to answer questions you might have about how to prepare and support your child during the procedure. A child life specialist can also be present at the time of your child's appointment to explain the procedure in developmentally appropriate ways using well established preparation materials. What should you do after the exam?

If your child received contrast, it is important to mildly increase their fluid intake for the day.

If your child received sedation, follow any instructions given by the sedation nurses upon completion of this procedure.

Test Results

The images from your childs exam are interpreted on the same day and a report sent to your physician's office.

 

 

Computed Tomography (CT)      for parents

What is Computed Tomography (CT)?

Computed Tomography (CT), sometimes called a CAT Scan, uses x-rays and computers to create an image of the inside of the body. Most body parts can be examined using CT.

The CT machine looks like a giant doughnut. The patient on the exam table moves through the opening of the doughnut.

The CT machine makes images that are different than a regular x-ray. It makes cross-sectional images of the body, most often referred to as slices. Just imagine looking at a piece of bread from the middle of the loaf. That's what a CT machine can do. The CT scanner takes pictures, as the body moves through the machine, of different sections or slices of tissue.

What should you do prior to the exam?

There are different preparations to follow according to exactly what type of CT scan your child is having.

If your child is:

  • Not being sedated and is not receiving radiographic contrast, then there are no special preparations for your child to follow.
  • Having a CT of the abdomen or pelvis with contrast, there is often a special oral prep for your child to follow. A prep kit needs to be obtained from the CT department ahead of time. Instructions on how to take the prep are explained and included. The preps are specific to the study being performed and your child's age. If you have questions about the prep kit, please call the CT department at 215-590-2399.
  • Being sedated for the exam,. Sedation is required for all children ages 6 months - 3 years.
  • Not being sedated but receiving IV radiographic contrast for the exam, they may have nothing to eat for 2 hours prior to the exam. Clear liquids are allowed up to the time of the exam.

If your child has a known allergy to radiographic contrast, additional preparations may be necessary. Please contact your referring physician to ask about these preparations if necessary.

Dress your child comfortably, in clothes that are easily removed (Sweat clothes, t -shirts). Your child may be given a gown to change into for the CT scan, if required.

Limit the use of jewelry and/or metal (zippers, snaps) in the area of scanning interest.

If you have copies of your child's previous imaging studies from another institution, please bring them for comparison.

Note: Parents/guardians will be allowed to accompany the child into the exam room; other arrangements should be made for siblings.

Women who are pregnant or may be pregnant will be asked to leave the exam room during the procedure. Please make sure that there is someone else available to accompany the child during the exam, if needed.

What should you do when you arrive?

If your appointment is scheduled at the Main Hospital:

You must check in to Outpatient Registration, on the third floor of the Main Hospital, 45 minutes prior to your scheduled appointment. This is where your registration process occurs.

The law requires that you bring a prescription from your doctor's office stating the exam to be done and the reason for it.

If your insurance requires a referral, it must be presented at the time of registration. Proof of insurance will also need to be provided at this time.

Once the registration process is completed, the CT Department will be notified. A technologist or nurse will come out to the central registration area and bring you and your child to a room for IV placement if necessary.

If your appointment is scheduled at the Pediatric Imaging Center at King of Prussia:

You must check in at the main registration desk, located in the main waiting room, 45 minutes prior to your scheduled appointment. This is where your registration process occurs.

The law requires that you bring a prescription from your doctor's office stating the exam to be done and the reason for it.

If your insurance requires a referral, it must be presented at the time of registration. Proof of insurance will also need to be provided at this time.

Once the registration process is completed, you will be directed to the Radiology waiting room. The CT technologist will be notified and a technologist or nurse will come out to the Radiology waiting room and bring you and your child to a room for IV placement if necessary.

What should you expect during the exam?

If your child is receiving contrast and/or  you will first be taken to our nursing station. Here, a nurse will  in your child for the injection needed for the exam. This IV tube will be placed in the child's arm, hand, or foot. Your child will only feel a little discomfort. Once the child has proper IV access, and/or a sedation assessment completed, the procedure can begin.

You and your child will be taken to the CT exam room. The technologist will ask why the CT is being performed and explain the procedure to both you and your child. Procedures may vary depending on the part of the body being examined.

The technologist will position your child on the CT table, most likely lying on their back, depending on the body part being examined. The CT scanner has a red light that will shine on your child's body to help ensure that your child is in the right position. A lead shield will be provided for your child, as long as it does not interfere with the exam. If you are accompanying your child into the room, a shield will be provided to you as well.

The technologist will ask your child to hold very still during the scan. Any type of movement during a CT scan will make the images look fuzzy, and may have to be repeated. Your child may also have to hold his/her breath a couple times during the scan.

After the technologist leaves the room, the CT scan will begin. The table will move through the doughnut hole slowly, and may even peek out the other side! The CT scanner makes a small noise like the whirl of a washing machine. A voice recorded in the CT scanner will tell your child when to hold his/her breathe, and when they can breathe again.

Your child will not feel the x-rays from the CT scan, but sometimes holding still for a long time can be uncomfortable. The CT scan only takes about 5 to 15 minutes; however, your child may be in the room longer while the images are checked for quality, or if additional images are needed.

The images are displayed on a computer screen and are sent to the Radiologist to be read.

What is General Radiography (X-ray)?

General radiography is the most basic form of medical imaging. It uses x-rays to create a fixed or still image of the inside of the body. Any part of the body can be examined.

The image created is viewed and interpreted by a doctor, called a radiologist, to detect and diagnose medical conditions in the body.

Several different terms, such as "film", "picture", and/or "x-ray", are used to describe the image produced.

After reading (interpreting) an image, the radiologist will dictate a report that will be sent to your referring physician. The radiologist may recommend further investigation through the use of other imaging methods.

What is an x-ray?

An actual x-ray is a form of energy. Depending on the energy level of an x-ray, it can either penetrate through the body, or is absorbed. The body and its tissues have many different densities, which will affect how an image appears. Areas such as bone are not easily penetrated; therefore, they appear white on an image. Areas such as the lungs, which are mainly air, are penetrated very easily; therefore, they appear black or grayish on an image. You cannot feel an x-ray as it goes through the body.

The x-ray machine consists of either a long table with a camera on top of it, or a large plate against the wall, with the camera pointing at it. The camera does not touch the patient but it may come close. There is a light, which will shine from the camera, to help the technologist position the patient to get the best "picture" possible. It is just like getting your picture taken at home!

What should you do prior to your child's exam?

What should you expect during the exam?

The technologist will ask why the x-ray is being performed and explain the procedure to both you and your child. Procedures vary depending on the part of the body being x-rayed. Most procedures require the technologist to take 2 to 3 images in different positions or from different angles.

The technologist will position the patient and point the camera at the body part being examined. The light from the camera will shine on your child's body to help the technologist aim the x-rays. A lead shield will be provided for your child as long as it does not interfere with the exam. If you are accompanying your child into the room, a shield will be provided to you. The technologist will ask your child to hold still and, sometimes, to hold his/her breath for a second. You may be asked to assist holding your child still during the exam. Any type of movement during an x-ray will show up blurry. The images appear on a computer screen and are sent to the radiologist to be read.

Your child will not feel the x-rays, but sometimes the required positions for the area being x-rayed are awkward or uncomfortable. These positions only have to be held for a couple of seconds.

        What It Is

A voiding cystourethrogram (VCUG) uses a small amount of radiation to make images of a person's urinary system.

After the patient's bladder is filled with a liquid called contrast material, an X-ray machine sends beams of radiation through the abdomen and pelvis, and images are recorded on special film or a computer. These images help doctors see problems in parts of the urinary system, including the bladder, urethra (the tube connecting the bladder with the outside of the body), and the ureters (the tubes connecting the kidneys to the bladder).

X-ray images are black and white. Dense body parts that block the passage of the X-ray beam through the body, such as bones, appear white on the X-ray image. Contrast material also appears white, and when it fills the organs of the urinary system, it makes them visible. Softer body tissues, such as the skin and muscles, allow the X-ray beams to pass through them and appear darker. Air in the stomach and intestines appears black.

An X-ray technician or radiologist takes the X-rays using a technique called fluoroscopy. While the contrast material fills your child's bladder, and then while your child empties the bladder, the technician or radiologist watches an onscreen X-ray video of the liquid moving through the urinary system and a series of X-ray films is recorded.

Why It's Done

A VCUG can check for problems in the structure or function of the urinary system. It can evaluate the bladder's size and shape and look for abnormalities, such as a blockage along the path of the urine.

It also can show whether the urine is moving in the right direction. Normally, urine flows from the kidneys down to the bladder through the ureters. When urine goes back up toward the kidneys, it's called vesicoureteral (VU) reflux, which a VCUG can detect. Sometimes VU reflux only occurs while urinating (voiding), which is why the VCUG includes taking X-ray images while the bladder is being emptied.

Since VU reflux can cause urinary tract infections (UTIs), this test is sometimes recommended after a child has had a UTI. Although not all kids who have had UTIs have reflux, it's important to find those who do, since the treatment may vary depending on the severity of reflux.

Radiographic Evaluation of Hallux Valgus

Hallux valgus is a common foot disorder of several etiologies, which can lead to significant foot pain and deformity. Little has been published in the radiographic literature about the pre- and postoperative radiographic findings of this very common and very treatable cause of foot pain.

Definition

The term hallux valgus denotes deviation of the great toe toward the fibular border of the foot. Hallux valgus is not synonymous with bunion, which is derived from the same root as "bun" or "bunch", and means an area of swelling. In connection with the foot, bunion usually refers to the prominent medial portion of the first metatarsal head and especially to the bursa or a bursa plus osteophyte over it, when this exists. A bursa and/or osteophyte may or may not accompany hallux valgus.

Pathogenesis

The etiology of hallux valgus is somewhat controversial. Some cases are congenital, perhaps secondary to a sloping surface of the first tarsometatarsal joint. When this joint is hypermobile, with or without the normal angle, it is often referred to as an "atavistic" tarsometatarsal joint. Other cases are almost certainly due to environmental factors, such as poorly fitting footwear. The fashionable shoes worn by many women are more constraining than the shoes worn by men and are felt by many authors to be the etiologic factor in most cases of hallux valgus. This would help to explain the 10:1 ratio of females to males seen with this disorder.

Flats:

Same patient as figure above, but now in shoes with 3 inch heels. The intermetatarsal angle has widened in both feet, and a functional hallux valgus has developed, due to her constricting shoes.

Radiographic Findings

Hallux valgus is often associated with abnormalities in two planes. In such cases, the first metatarsal head is not only deviated medially, but also dorsally. As the first metatarsal splays dorsally, greater stress is placed on the central metatarsals, especially the second, leading to hyperostosis and occasionally stress fractures.

Foot with metatarsus primus varus. Dorsal splaying of both the first and fifth metatarsal heads is noted. Keratosis (dark pink) is seen in the skin adjacent to these heads, due to pressure against the shoe. The central metatarsal heads now bear more of the weight, leading to plantar keratosis and hyperostosis.

Normally, the 1st and 2nd metatarsals are parallel to each other, and their superior surfaces appear within 1 - 2 mm of each other, as shown here.

MPVLatFoot:

Some patients with metatarsus primus varus demonstrate not only medial angulation of the first metatarsal head, but also dorsal angulation as well. Here the first metatarsal is elevated several mm above the second metatarsal head.

Sesamoid:

Two large sesamoid bones are present beneath the first metatarsophalangeal joint within the tendons of the flexor hallucis brevis. With the development of hallux valgus, the 1st metatarsal head migrates medially and dorsally. The fibular sesamoid frequently rotates slightly dorsally, and is seen on the AP film in profile.


BilatHV:

This patient has a marked hallux valgus deformity in both feet. As the first toe migrates more and more to valgus, it presses against the second toe, and may aggravate dorsal clawing of the second toe, which is usually initiated by overload of the second metatarsal and synovitis of the metatarsophalangeal joint. Also note the relative hyperostosis of the second metatarsal shafts compared to the third and fourth, as well as dorsal rotation of the fibular sesamoids.

CalBursitisDetail:

This patient exhibits soft tissue swelling medial to her first metatarsal head, indicative of bursitis and synovial thickening. Also noted is dystrophic calcification in this bursa. Such changes could also be seen in tophaceous gout.

PreOp:

Preoperative film showing hallux valgus and clawtoe deformities of the second and third toes. Medial subluxation of the head off the sesamoids is shown, as well as marked hyperostosis of the second metatarsal shaft.

PostOp:

Same patient status post Lapidus procedure. With osteotomy of the 1st metatarsal, the 1st metatarsal head has been realigned in a more lateral and plantar location over the sesamoids. The 1st tarsometatarsal joint has been fused. Kirschner wires are seen in the 2nd and 3rd toes after osteotomy for clawtoe deformities.

Broken:

Broken screw following Lapidus procedure. Cancellous screws, especially the thinner ones, usually break at the junction of the shank and the threaded portion. Therefore, this probably represents at least a delayed union if not a nonunion. However, if correct position is maintained, as in this case, and the surgical site is painless, it may be considered a satisfactory result.

HVarus:

This patient has undergone bilateral osteotomy of the medial 1st metatarsal heads ("bunionectomy"). Unlike the Lapidus procedure, this procedure does not correct the underlying splaying of the 1st metatarsal, and can lead to instability of the first metatarsophalangeal joint. In this case, the hallux valgus has been overcorrected, and the patient now has a bilateral hallux varus. Bilateral osteotomies are noted in the 5th metatarsals for correction of "bunionette" deformities.

CONCLUSIONS



  1. Hallux valgus is a very common and very treatable cause of foot pain.

  2. Hallux valgus cannot be adequately assessed unless weight-bearing views are performed in the AP and lateral positions.

  3. With experience, the radiologist can make valuable observations in both the pre- and postoperative films in patients with hallux valgus.

احتمالاً در بیمارستان یا حداقل در فیلم های تلویزیون بیمارانی را دیده اید که برای درمان سرطانشان تحت پرتو درمانی قرار می‌گیرند و یا اینکه پزشکان برای تشخیص بیماریها دستور عکس برداری PET را صادر می‌کنند. همه اینها قسمتی از علم پزشکی هستند که به طور خاص به آن پزشکی هسته ای می‌گویند. در پزشکی هسته ای برای مشاهده اعضای بدن و درمان بیماریها از مواد رادیواکتیو استفاده می‌شود. در پزشکی هسته ای برای تشخیص و درمان بیماریها، هم فیزیولوژی ( بررسی عملکرد ) و هم آناتومی بدن بررسی می‌شود.
خوب، حالا می‌خواهیم برخی از تکنیک هایی را که در پزشکی هسته ای استفاده می‌شود توضیح دهیم. و ببینیم که پرتوها چطوری به پزشکان کمک می‌کنند تا اعماق بدن انسان را ببینند.

تصویر برداری در پزشکی هسته ای
مشکل تصویر برداری از بدن انسان این است که ماده ای کدر و غیر شفاف است، نگاه کردن درون بدن انسان نیز بطور کلی دردناک است. در گذشته روش معمول دیدن درون بدن انسان جراحی بود! اما امروزه با استفاده از انبوهی از روشهای جدید دیگر نیازی به این روشهای وحشتناک نیست. تصویر برداری اشعه X، MRI، تصویر برداری CAT و مافوق صوت برخی از این تکنیک‌ها هستند. هر کدام از این تکنیک‌ها مزایا و معایبی دارند که باعث می‌شود برای شرایط مختلف واعضای مختلف بدن مفید باشند.
تکنیک های تصویر برداری پزشکی هسته ای روشهای جدیدی را برای نگاه کردن به درون بدن انسان برای پزشکان فراهم می‌کند. این تکنیک‌ها ترکیبی از استفاده از کامپیوتر، حسگرها و مواد رادیواکتیو است. این روشها عبارتند از:
• توموگرافی با استفاده از تابش پوزیترون (PET)
• SPECT
• تصویر برداری قلبی - عروقی
• اسکن استخوان
هر کدام ازاین روشها از یکی از خصوصیات عناصر رادیواکتیو برای تولید یک تصویر استفاده می‌کنند.
تصویر برداری در پزشکی هسته ای برای شناسایی موارد زیر بسیار مفید است:
• تومورها
• Aneurysms آنوریسم
• نارسایی سلول های خونی و اختلال در عملکرد دستگاههای بدن مثل غده تیروئید و ریه
استفاده از هر کدام از این روشهای خاص یا مجموعه ای از آنها بستگی به علائم بیمار و نوع بیماری دارد.

توموگرافی تابش پوزیترون (PET)
PET با استفاده از تابش های ساطع شده از مواد رادیواکتیو تصاویر قسمتهای مختلف بدن را تولید می‌کند. مواد رادیواکتیو به درون بدن تزریق می‌شوند و معمولاً به دام اتمهای رادیواکتیو مثل کربن -11، فوئور -18، اکسیژن -15 و یا نیتروژن -13 که نیمه عمر کوتاهی دارند، گرفتار می‌شوند. این اتمهای رادیواکتیو ایزوتوپهای رادیواکتیو اتمهای طبیعی هستند که عمر کوتاهی دارند. با بمباران اتمهای طبیعی به وسیله نوترون می‌توان این اتم‌ها را تولید کرد. وقتی مواد رادیواکتیو تزریق شده به بدن با الکترونهای درون سلول برخورد می‌کنند، پوزیترون اشعه گاما تولید می‌شود. در روش PET با دنبال کردن این اشعه های گاما تصویر برداری انجام می‌شود.
در یک PET اسکن همانطور که گفتم ابتدا به بیمار مواد رادیواکتیو تزریق می‌شود، سپس بیمار روی یک تخت صاف دراز می‌کشد. این تخت به درون یک اتاقک استوانه ای شکل وارد می‌شود، در دیواره های این اتاقک دنبال کننده های اشعه گاما به صورت آرایه دایره ای شکل قرار گرفته اند. این دنبال کننده‌ها یک سری کریستالهای Scintillation دارند که هر کدام به یک تقویت کننده نوری متصل است. این کریستالها اشعه های گامای ساطع شده از بیمار را به فوتون های نور تبدیل می‌کنند تقویت کننده نوری این فوتونها را به سیگنالهای الکتریکی تبدیل کرده و آنها را تقویت می‌کند. کامپیوتر این سیگنالها را پردازش کرده و تصویر را تشکیل می‌دهد. سپس تخت بیمار جا به جا شده واین فرآیند تکرار می‌شود. در نتیجه یک سری تصویر از عضوی که در آن تزریق شده ( مثل مغز، سینه، کبد و ... ) به دست می‌آید این تصاویر کنار هم قرار می‌گیرند تا یک تصویر سه بعدی از عضو مورد نظر به وجود آید.
PET می‌تواند تصاویری از جریان خون ودیگر فعالیت های بیوشیمیایی بدن، بسته به این که چه نوع مولکولی به دام اتمهای رادیواکتیو افتاده است، تهیه کند. به عنوان مثال PET می‌تواند تصاویری از متابولیسم گلوکز در مغز تهیه کند. با این حال مراکز PET کمی در دنیا وجود دارد چون این مراکز باید در کنار یک شتابدهنده ذرات ساخته شوند تا بتوان رادیوایزوتوپهای مورد استفاده در این روش را تأمین کرد.

(SPECT) توموروگرافی با استفاده از تابش تک فوتون
SPECT روشی بسیار شبیه به PET است با این متفاوت که ایزوتوپهای مورد استفاده در این روش ( که عبارتند از زنون - 133، تکنتیوم - 99 و لودین - 123 ) زمان واپاشی طولانی تری دارند و به جای تابش 2 اشعه گاما فقط یک اشعه گاما تابش می‌کنند. این روش نیز می‌تواند اطلاعاتی در مورد جریان خون و پراکندگی موارد رادیواکتیو در بدن ارائه دهد، البته تصاویر آن حساسیت کمتری دارند و جزئیات کمتری را نسبت به تصاویر PET نشان می‌دهند. اما مزیت مهم این روش نسبت PET این است که به گرانی روش PET نیست. در ضمن تعداد مراکز SPECT بیشتر از مراکز PET هستند، چون در این موارد دیگر نیازی نیست که مراکز در کنار یک شتاب دهنده ساخته شوند.

تصویر برداری قلبی عروقی
در این تکنیک از مواد رادیواکتیو برای مشخص کردن جریان خون در قلب و رگهای خونی استفاده می‌شود. مثال خوب برای این تکنیک آزمایش تنش تالیوم است در این آزمایش یکی از ترکیبات رادیواکتیو تالیوم به بیمار تزریق می‌شود بیمار یک سری نرمش انجام می‌دهد و به وسیله دوربین های پرتو گاما از قلب بیمار عکس برداری می‌شود. پس از یک استراحت مطالعات دوباره تکرار می‌شود؛ اما این بار بدون فعالیت بدنی. تصاویر گرفته شده قبل و بعد از نرمش کردن با هم مقایسه می‌شوند تا تغییرات جریان خون مشاهده شود. این روش برای تشخیص تصلب شراین در قلب و دیگر اعضا مناسب است.

اسکن استخوان
در این روش تابش های مواد رادیواکتیو ( تکنتیوم - بی پی متیل دی سولفات ) تزریق شده به بدن که در بافت استخوان جمع شده اند، آشکار می‌شوند. بافت استخوان ترکیبات فسفر را به خوبی در خود جمع می‌کند. این مواد در نقاطی که فعالیت متابولیک بالایی دارند بیشتر جمع می‌شوند. بنابراین تصویر گرفته شده یک سری نقاط روشن که نشان دهنده فعالیت بالا هستند و یک سری نقاط تاریک که نشان دهنده فعالیت پایین هستند را نشان می‌دهد. اسکن استخوان روش خوبی برای تشخیص تومورهاست. تومورها بطور کلی فعالیت متابولیک بالایی دارند.

از مواد رادیواکتیو به عنوان ردیاب رادیواکتیو استفاده می‌شود. این مواد از طریق بلعیدن و یا تزریق وارد جریان خون می‌شود. یکی از روشهای ردیابی به این شکل است که مواد ردیاب در خون حرکت می‌کنند و امکان می‌دهند که ساختار رگهای خونی مشاهده شود. این روش مشاهده به پزشکان این امکان را می‌دهد که لخته و دیگر ناهنجاریهای رگهای خونی را به راحتی تشخیص دهند. علاوه بر این، برخی اعضاء بدن هستند که نوع خاصی از مواد شیمیایی را در خود جمع می‌کنند . برای مثال غده تیروئید ، ید را در خود جمع می‌کند بنابراین با بلعیدن ید رادیواکتیو ( به صورت مایع یا به صورت قرص ) می‌توان تومورهای تیروئید را تشخیص داد و درمان کرد. به همین ترتیب تومورهای سرطانی نیز، فسفات را در خود جمع می‌کنند. بنابراین با تزریق ایزوتوپ رادیواکتیو فسفر - 32 در جریان خون می‌توان تومورهای سرطانی را، به دلیل افزایش رادیو رادیواکتیوشان، شناسایی کرد.
در تصویر برداری، آزمایش یا درمان به وسیله پزشکی هسته ای، مواد رادیواکتیوی که بلعیده یا تزریق می‌شوند به بدن آسیب نمی رسانند. رادیو ایزوتوپ هایی که در پزشکی هسته ای استفاده می‌شوند به سرعت در عرض چند دقیقه تا حداکثر یک ساعت واپاشیده می‌شوند. سطح تابش های رادیواکتیو آنها هم نسبت به اشعه X یا CT اسکن بسیار پایین تر است.
برخلاف درمان از طریق پزشکی هسته ای، رادیوتراپی ( که کاملاً با آن متفاوت است ) از این مزیت بهره می‌گیرد که برخی سلولها با شدت بسیار بیشتری تحت تأثیر تابش های یونیزه یعنی تابش های آلفا، بتا و گاما و X قرار می‌گیرند. سلولها با سرعت های متفاوتی تقسیم می‌شوند و سلولهایی که با سرعت بیشتری تقسیم می‌شوند به دو دلیل، بیشتر تحت تأثیر تابش های یونیزه قرار می‌گیرند:
- سلولها دارای مکانیسمی هستند که به آنها این امکان را می‌دهد تا DNA آسیب دیده را ترمیم کنند.
- وقتی که یک سلول در حال تقسیم متوجه شود که DNA آسیب دیده است خودش را از بین می‌برد.
سلولهایی که به سرعت تقسیم می‌شوند زمان کمتری برای مکانیسم ترمیم و شناسایی خطاهای DNA قبل از تقسیم شدن دارند، بنابراین احتمال بیشتری وجود دارد که پس از قرار گرفتن در معرض تابش های هسته ای از بین بروند.
از آنجایی که در اکثر انواع سرطان، سلولهای سرطانی به سرعت تقسیم می‌شوند در برخی موارد می‌توان به وسیله رادیوتراپی سرطان را درمان کرد. معمولاً مواد رادیواکتیو اطراف یا کنار تومور قرار می‌گیرند. در تومورهایی که در عمق بدن یا نواحی غیر جراحی قرار گرفته اند پرتو X با شدت بالایی روی تومور تابانیده می‌شود.
اما تنها مشکلی که این نوع از درمان دارد این است که دیگر سلولهای سالم که به سرعت تقسیم می‌شوند نیز، همراه سلولهای سرطانی تحت تأثیر پرتوها قرار می‌گیرند. به همین دلیل کسانی که تحت درمان سرطان هستند دچار حالت تهوع و ریزش موی شدید می‌شوند.

ساختار اتم

مقدمه:

اتم در زبان يونانى به معنى تقسيم ناپذير است. اين ايده، زاده تفكر دموكريتوس فيلسوف يونانى در ۲۳۰۰ سال پيش است. براى او اين تصور محال بود كه اجسام مادى بتوانند بى حد و حصر تقسيم شوند. اما «جان دالتون» شيميدان بود كه نخستين نظريه اتمى نوين را ارائه كرد. دالتون كه كارش پژوهش در مورد هواشناسى بود، به تركيب گازها علاقه مند شد و خيلى زود ايده تشكيل گازها از واحدهاى كوچك غيرقابل تقسيم در ذهنش شكل گرفت. او اين نظريه را در سال ۱۸۰۸ تحت عنوان «سيستم جديد فلسفه شيمى» منتشر كرد. تا دهه پايانى قرن نوزدهم دو جنبه اساسى فيزيك كلاسيك يعنى مكانيك كلاسيك و الكترومغناطيس به خوبى شناخته شده بود و دانشمندان گمان مى كردند كه طبيعت براساس دو نيروى گرانشى و الكترومغناطيسى ساخته شده است. درست در همين زمان بود كه پديده هايى مشاهده شد كه طى دهه هاى ابتدايى قرن بيستم منجر به بزرگترين انقلاب هاى تاريخ علم يعنى نسبيت عام و مكانيك كوانتومى شدند.

راديواكتيويته
در سال 1896 آنتوان هانرى بكرل
 Becquerel که فيزيكدان فرانسوى كه از كشف اشعه ایکس به وسيله رونتگن مطلع شده بود، به دنبال يك رشته آزمايش روى سنگ معدنى به نام اورانيل، فعاليت هاى پرتوافشانى خود به خودى خاصى را كشف كرد و آن را «راديواكتيويته» نام گذاشت. پس از او مارى و پى ير كورى هم دو عنصر راديوم و پولونيوم را كشف كردند كه خاصيت راديواكتيويته بسيار بيشترى داشتند. اما بيشتر پژوهش ها روى راديواكتيويته به وسيله لرد رادرفورد انجام شد. او كشف كرد كه خاصيت راديواكتيويته ناشى از پراكنش سه نوع اشعه است:

۱- اشعه آلفا كه توسط يك برگ كاغذ متوقف مى شود. بار آن مثبت است و در حقيقت همان يون هاى هليوم دو بار مثبت يا هسته اتم هليوم است.
۲- اشعه بتا كه از ورقه چند ميلى مترى آلومينيوم رد مى شود. بار آن منفى است. ماهيت اين اشعه الكترون هاى پرانرژى است.
۳- اشعه گاما كه از صفحات سربى به ضخامت ده ها سانتى متر هم عبور مى كند، از لحاظ الكتريكى خنثى است. اين اشعه فوتون هاى پرانرژى با طول موج بسيار كوتاه است.
دانشمندان با توجه به مجموعه آزمايش هاى رادرفورد به اين نتيجه رسيدند كه اتم ها برخلاف نامشان از اجزاى كوچكترى هم تشكيل شده اند.

هسته

افتخار كشف هسته اتم نيز از آن رادرفورد است. او با كمك دو دانشجويش به نام گايگر و مارسدن با انجام آزمايشى كه «پراكندگى» نام دارد، به وجود هسته پى برد. رادرفورد فكر مى كرد كه اتم ها مثل مدل كيك كشمشى تامسون از تعدادى الكترون تشكيل شده اند كه در يك فضاى پيوسته با بار مثبت قرار دارند. به همين دليل ذرات آلفا را به سمت ورقه  نازكى از طلا پرتاب كرد. اما پراكندگى اين ذرات از هسته طلا نشان داد كه بارهاى مثبت در ناحيه بسيار كوچكى در وسط اتم متمركز شده اند. شعاع اتم حدود يك آنگسترم (۱۰-10 متر) است ولى اندازه هسته حدود 10 فرمى (10-14 متر) است.

نيمه عمر

پس از اينكه رادرفورد ماهيت تشعشع راديواكتيو را كشف كرد، دانشمندان پى بردند كه راديواكتيويته به علت تلاشى خودبه خود هسته هاى سنگين و تبديل آنها به هسته هاى سبك تر است. در حين اين تبديل، ذرات آلفا، بتا و گاما ساطع مى شود. در حقيقت پس از خارج شدن اين ذرات از هسته، ماهيت آن تغيير مى كند. تعداد هسته هايى كه در هر لحظه متلاشى مى شوند با تعداد هسته ها در آن لحظه نسبت مستقيم دارد. زمانى را كه نيمى از هسته هاى ماده ابتدايى متلاشى مى شوند، نيمه عمر ماده مى گويند. يعنى اگر در ابتدا يك گرم ماده راديواكتيو داشته باشيم، پس از يك نيمه عمر نصف و پس از دو نيمه عمر، يك چهارم و پس از سه نيمه عمر، يك هشتم مقدار اوليه را خواهيم داشت. نيمه عمر مواد مختلف متفاوت است و از چند ميليارديوم ثانيه تا چندين ميليارد سال تغيير مى كند. معمولاً هرچه نيمه عمر بيشتر باشد، انرژى ساطع شده از تلاشى راديواكتيويته كمتر است.

نيمه عمر اورانيوم 4/5 ميليارد سال است. نيمه عمر راديوم 1590 سال و نيم عمر راكتانيوم كمتر از 10 هزارم ثانيه است.

درون هسته

 

مدل اتمى رادرفورد بيانگر اين مطلب بود كه هسته در وسط اتم داراى بار مثبت است و الكترون ها با بار منفى در اطراف آن قرار دارند. مدل اتمى بور هم مدل رادرفورد را كامل كرد و سازوكار منظمى را براى استقرار الكترون ها در اطراف هسته تدوين كرد. اما تفسير و توجيه راديواكتيويته ترديدى به جاى نمى گذارد كه هسته ها خود مجموعه  مكانيكى پيچيده اى هستند كه از اجراى سازنده متفاوتى تشكيل شده اند. اين واقعيت كه وزن اتمى ايزوتوپ هاى اتم هاى مختلف (بعضى از اتم ها درحالى كه جرم اندكى متفاوت با هم دارند، خواص شيميايى كاملاً يكسانى دارند، به اين اتم ها ايزوتوپ مى گويند.) با اعداد صحيح (يا لااقل بسيار نزديك به عدد صحيح) بيان مى شوند، نشان مى دهد كه پروتون ها (حاملان بار مثبت) بايد نقش يكى از اجزاى اصلى سازنده هسته را داشته باشند. ابتدا فرض مى كردند كه درون هر هسته علاوه بر پروتون، الكترون هم هست.

يعنى مثلاً كربن كه جرم 12 و بار ۶+ دارد، درون هسته خود 12 پروتون و ۶ الكترون دارد و علاوه بر آن در بيرون هسته هم ۶ الكترون به دور آن مى  چرخند اما اين راه حل از لحاظ نظرى مشكلات عديده اى را به همراه داشت. اما رادرفورد و بور پيشنهاد كردند كه علاوه بر پروتون ذره ديگرى هم جرم آن ولى بدون بار درون هسته است. آنها نام نوترون را براى آن انتخاب كردند و اين ذره در سال 1932 توسط چادويك كشف شد.

اسپين

اتم ها در اثر گرفتن انرژى، تابش مى كنند. اين تابش ناشى از اين است كه الكترون هاى اطراف هسته، انرژى مى گيرند و بعد اين انرژى را به صورت يك فوتون با طول موج معين بازمى تابانند. اما خود اين طيف در مجاورت ميدان الكترومغناطيسى، به چند طول موج جدا از هم تفكيك مى شود. علت اين است كه الكترون ها در اتم، اندازه حركت زاويه اى هم دارند. اشترن و گرلاخ نشان دادند كه الكترون ها علاوه بر اين اندازه حركت زاويه اى، خاصيت ديگرى هم دارند كه فقط در حضور ميدان مغناطيسى آن را بروز مى دهند. به دليل شباهت اين خاصيت به اندازه حركت زاويه اى، نام آن را «اندازه حركت زاويه اى ذاتى» يا اسپين نهادند. بعدها ثابت شد كه علاوه بر الكترون، باقى ذرات بنيادى هم اسپين دارند. مهمترين ويژگى اسپين اين است كه يك خاصيت كاملاً كوانتومى است و مشابه كلاسيك ندارد. ذراتى كه اسپين نيم صحيح دارند (يك دوم، سه دوم، ...) فرميون مى نامند، مثل الكترون، پروتون، نوترون و... اين ذرات تشكيل دهنده ماده هستند. در مقابل ذراتى كه اسپين صحيح دارند(صفر، ۱ ، ۲ و...) بوزون گفته مى شوند، مثل فوتون، مزون، گلوتون و... اين ذرات حامل نيروها هستند.

ايزواسپين و نيروى هسته اى

 

هنگامى كه نوترون توسط چادويك كشف شد، اين واقعيت مسلم شد كه علاوه بر نيروى گرانش و الكترومغناطيسى، حداقل يك نيروى ديگر در طبيعت وجود دارد و اين نيرو است كه عامل پيوند نوكلئون ها (پروتون ها و نوترون ها) درون هسته است. زيرا در صورت عدم وجود اين نيرو، در اثر دافعه شديد بارهاى مثبت پروتون ها بر هم، هسته از هم مى پاشد. از اين مثال برمى آيد كه اولاً اين نيرو بايد جاذبه اى باشد تا در مقابل دافعه پروتون ها بايستد و ثانياً برد آن بايد خيلى كوتاه باشد و از ابعاد هسته بيشتر نباشد. زيرا نيروى الكترومغناطيسى (در مدل بوهر) آرايش الكترون ها در مدارهاى اتمى را به خوبى توضيح مى داد. اما واقعيت مهم و جالب تر اين است كه بايد براى اين نيرو، پروتون و نوترون به يك شكل ديده شوند و فارغ از اختلاف بار الكتريكى اين دو ذره يك شكل باشند. هايزنبرگ با استفاده از اين واقعيت و با ايده گرفتن از نظريه اسپين، مفهوم رياضى جديدى به نام «ايزوتوپ اسپين» يا ايزواسپين را معرفى كرد. او پيشنهاد كرد كه همان طور كه در حضور ميدان الكتريكى خطوط طيفى يكى هستند و با ظهور ميدان مغناطيسى به چند خط ديگر شكافته مى شوند، نوكلئون ها (پروتون و نوترون) هم در حقيقت در مقابل نيروى هسته اى يك ذره هستند اما هنگام ظهور نيروهاى الكترومغناطيسى به دو ذره با ايزواسپين متفاوت تبديل مى شوند.

نيروى هسته اى قوى

يوكاوا فيزيكدان ژاپنى در سال ۱۹۳۵ براى توضيح نيروى هسته اى گفت: اين نيرو بايد در اثر مبادله ذره اى به نام پيون (مزون پى) بين نوكلئون ها به وجود بيايد. چون اين ذره نسبتاً سنگين است، اصل عدم قطعيت هايزنبرگ ايجاب مى كند كه برد اين نيرو كوتاه باشد، به اين ترتيب ايده مبادله ذره، توانست تمام ويژگى  هاى نيروى هسته اى را توضيح بدهد. پيون ها هم مثل نوكلئون ها براى نيروى هسته اى يك ذره به شمار مى روند اما ايزواسپين آنها يك است يعنى در مقابل نيروى الكترومغناطيسى ۳ حالت پيون با بار مثبت و با بار منفى و خنثى را دارند. يك پروتون، با از دست دادن يك پيون مثبت به نوترون تبديل مى شود و اين پيون مثبت خود يك نوترون ديگر را به پروتون تبديل مى كند. دوتا نوترون يا دوتا پروتون هم مى توانند با هم پيون خنثى (صفر) مبادله كنند. يك نوترون هم با از دست دادن يك پيون منفى به پروتون تبديل مى شود و اين پيون منفى با يك پروتون ديگر، يك نوترون توليد مى كند. به اين ترتيب با مبادله اين ذرات، نوكلئون ها در هسته پايدار مى مانند.

نيروى هسته اى ضعيف

يكى از ويژگى هاى بارز نوترون نيم عمر آن است. نوترون در حالت آزاد پس از ۱۸ دقيقه متلاشى و به يك پروتون و يك الكترون تبديل مى شود. اين مدت بسيار طولانى تر از تمام پديده هايى است كه با نيروى قوى سروكار دارد. نيرو هاى الكترومغناطيسى هم بر نوترون بدون بار عمل نمى كنند. پس واضح است كه تلاشى نوترون، ناشى از يك نيروى جديد در طبيعت است. به علت ضعيف بودن اين نيرو نسبت به نيروى هسته اى آن را نيروى هسته اى ضعيف نام گذاشتند. تلاشى هسته كه نتيجه آن توليد پرتو بتا است هم ريشه در اين نيرو دارد.

 شكافت
فرمى در فاصله كمى بعد از كشف نوترون در سال 1932 بررسى هسته اتم هاى سنگين بمباران شده به وسيله نوترون را آغاز كرد و از انجام اين آزمايش ها با اورانيوم نتايج عجيبى به دست آمد. اتوهان و اشتراسمن در سال 1939 اين معضل را حل كردند.

آنها كشف كردند وقتى كه اورانيوم با نوترون بمباران مى شود، هسته هايى مثل باريو توليد مى شوند كه عدد اتمى آنها خيلى كوچك تر از عدد اتمى اورانيوم است. ليز ميتنر فيزيكدان آلمانى كه در سوئد زندگى مى كرد، اين پديده را به دقت بررسى كرد و نام شكافت را براى آن انتخاب كرد. بور و ويلر با ارائه مقاله اى فهم نظرى شكافت را به طور كامل ممكن كردند و پس از ارائه مقاله آنها كليه پژوهش هاى علمى در مورد شكافت هسته اى تا به امروز جزء اسناد فوق العاده سرى، طبقه بندى مى شود.

گداخت

هسته هاى خيلى سبك مثل هيدروژن يا هليوم انرژى بستگى كمترى نسبت به هسته هاى سنگين دارند. اگر دو هسته سبك در هم ادغام شوند، هسته سنگين ترى را به وجود مى آورند و مقدار زيادى انرژى به صورت انرژى جنبشى آزاد مى شود. براى انجام گداخت بايد هسته ها را بسيار به هم نزديك كرد. دافعه الكترواستاتيكى مانع بزرگى براى اين فرآيند است. اين واكنش با افزايش انرژى جنبشى هسته هاى اوليه انجام مى شود. دسترسى به چنين انرژى هايى در شتاب دهنده ها آسان است اما براى اينكه اين واكنش خودنگهدار باشد، به دمايى حدود ۱۰۸ كلوين نياز است. (دماى سطح خورشيد شش هزار كلوين است.) چنين وضعيتى تنها در حالت پلاسمايى ماده پيش مى آيد كه در آن هسته ها و الكترون ها از هم جدا هستند. پژوهش ها به روى گداخت هسته اى همچنان ادامه دارد و قرار است در رآكتور Iter در فرانسه براى نخستين بار چنين فرآيند خود نگهدارى اى ايجاد شود. اما شايد رسيدن به اين هدف چند دهه طول بكشد.

رادیواکتیویته (Radioactive)

مواد رادیواکتیو از اتم های ناپایداری تشکیل می شوند که تجزیه می شوند و انرژی سطح بالایی به نام تابش رادیواکتیو را آزاد می کنند این اتمها نهایتا عناصر جدیدی را تشکیل می دهند. سه نوع تابش رادیواکتیو وجود دارد که ذرات آلفا ، ذرات بتا ، و پرتوهای گاما خوانده می شوند.

اطلاعات اولیه:

پرتو آلفا (دو پروتون و دو نوترون): جرم چهار واحد اتمی (a.m.u) و بارالکتریکی مثبت در پرتو بتا (الکترونهای سریع): جرم ناچیز و بارالکتریکی منفی یک و پرتو گاما (موج الکترومغناطیسی): بدون جرم و بدون بار (مثلا انرژی خالص)
تاریخچه: حدود اواخر قرن نوزدهم اکثر دانشمندان بر این عقیده بودند که تمام مسائل عمده فیزیک حل شده اند ، به غیر از چند مورد جزئی برای قطعیت دادن به برخی نظریه های ضروری بود. در سال 1895 ، رزتگن اشعه ایکس را کشف کرد. این اشعه نخست در معاینات پزشکی به کار رفت و بعدها برای بررسی ساختمان اساسی مواد مورد استفاده قرار گرفت چند ماه بعد ماری کوری این پدیده جدید را رادیو اکتیو نامید. او و شورش پی یر کوری ، همچنین پولونیم (po ، فلز ضعیف) و رادیم (Ra ، فلز قلیایی خاکی) را کشف کردند. ماری کوری نخستین کسی بود که از اصطلاح «رادیواکتیو» برای موادی که فعالیت الکترومغناطی قابل توجه دارند استفاده کرد. خاصیت رادیواکتیویته این دو عنصر جدید از اورانیم بیشتر بود.

سیر تحولی و رشد:

ماری کوری تحقیق خود را با جستجوی کاربردهای پزشکی رادیواکتیو ادامه داد. و قدرت تشعشع ترکیبات اورانیم را اندازه گرفت و تحقیق خود را به عناصر دیگر از جمله توریم ، گسترش داد.
  • در سال 1934 میلادی زوج ژولیو- کوری رادیواکتیویته مصنوعی را کشف کرد.
  • ماری کوری پی یر کوری همراه با فیزیکدان فرانسوی هانری بکرل (1908-1852 م) مدل دیوی انجمن سلطنتی انگلستان و جایزه نوبل را در فیزیک برای کشف رادیواکتیو دریافت دریافت می کنند. پی یر کوری کشف می کند که رادیم Ra خود بخود حرارت آزاد می کند. این خاصیت نمود ثبت شده از انرژی اتمی به شکل گرماست.
در سال 1910 میلادی در کنفرانس بروکسل در مورد رادیواکتیویته ، واحد رادیواکتیویته به افتخار او کوری نامیده شد. در مورد کشف رادیواکتیویته توسط هانری بکرل باید بگوییم که در سال 1896 میلادی ، بکرل در جستجوی شواهدی بود که ثابت کند مواد شیمیایی که نور طبیعی فلوئورسان هستند از خود پرتو ساطع می کنند.
او یک نمونه سولفات پتاسیم اورانیم را برداشت و آن را همراه با یک صفحه عکاسی در کاغذ سیاه پیچید. از آنجا که روزی ابری بود. نمونه بکرل خاصیت فلوئورسانی را از خود نشان نمی داد. او آن را درکشویی در آزمایشگاه خود گذاشت و به آزمایشهای خود در مورد لامپهای اشعه کاتدی ادامه داد. چند روز بعد ، دریافت که نمونه تصویری را بر روی صفحه عکاسی ایجاد کرده است. این نشان می داد که ماده مذکور شکلی از تشعشع را که بعدا ماری کوری آن را رادیواکتیویته نامید ، از خود ساطع کرده است.1922 میلادی نیلز بور نظریه طیفهای ساختار اتمی را منتشر کرد و در 1927 میلادی اصل مکمل بودن را تنظیم می کند که رفتار پیچیده رادیواکتیویته را توصیف می کند.
ارنست رادرفورد فیزیکدان بریتانی نیوزلندی الاصل (1871-1937) بر روی رادیواکتیویته و ماهیت ذرات آلفا (دارای بار مثبت) تحقیق کرد و متوجه شد که بار مثبت اتم در مرکز آن و در هسته ای ریز و متراکم متمرکز است. در سال 1930 میلادی رادرفورد تشعشعات مواد رادیواکتیو را منتشر کرد.

تابشهای رادیواکتیو:

چنان که گفته شد سه نوع تابش رادیواکتیو وجود دارد که ذرات آلفا از چهار ذره اتمی ، یعنی دو پروتون و دو نوترون تشکیل می شوند. این ذرات ضعیفترین نوع تابش رادیواکتیو هستند. و بار الکتریکی مثبت دارند. مسیر آنها را می توان با صفحه کاغذ مسدود کرد. ذرات بتا قدرتمند و از ذرات اتمی که الکترون خوانده می شوند و بار منفی دارند تشکیل می شوند. این کاغذ عبور می کند ولی آلومینیوم آن را مسدود می کند. پرتوهای گاما از همه قدرتمند ترند. آنها امواج الکترومغناطیسی اند و فاقد بارالکتریکی می باشند. اما پرتوهای گاما را فقط لایه ضخیمی از سرب متوقف می سازد. خروجی یا تابش رادیواکتیو می تواند وارد بافتهای زنده شود و به آنها صدمه بزند. بنابراین اطراف آن باید کنترل شود. این تابش را با وسیله ای به نام شمارنده گایگر – مولر ، که نام آن از مخترعانش اقتباس شده است ، می توان اندازه گرفت. وقتی تابش رادیواکتیو وارد این شمارنده می شود ، گاز موجود در آن حامل الکتریسیته می شود. مقدار بار را می توان روی صفحه ای قرائت کرد یا از طریق یک بلند گو به صورت صداهای تیک تیک خاصی شنید.

نیمه عمر:

نیمه عمر یک ماده زمانی است که طول می کشد تا خاصیت رادیواکتیویته آن به نصف کاهش یابد. مثلا نیمه عمر کربن 14 (شکل خاصی از عنصر کربن) 5600 سال است. یعنی 5600 سال طول می کشد تا نصف اتم های رادیواکتیو کربن دچار فروپاشی شوند ، یا یک گرم از اتم های رادیواکتیو به نیم گرم تقلیل یابد. 5600 سال دیگر طول می کشد که همین مقدار نیز به نصف برسد و به همین ترتیب.
نیمه عمر عناصر مختلف از چند ثانیه تا میلیونها سال متغیر است. فروپاشی شبکه ای زباله های اتمی زیان بخش حاصل از نیروگاههای هسته ای میلیونها سال طول می کشد. و همه موجودات زنده روی زمین حاوی مقدار معینی کربن 14 (کربن رادیواکتیو) هستند که با تبادل مداوم گازهای اکسیژن و دی اکسید کربن بین موجودات زنده و جو زمین تشکیل می شود. وقتی یک گیاه یا حیوان می میرد ، این تبادل متوقف می شود و کربن 14 شروع به فروپاشی می کند.
دانشمندان می دانند که نیمه عمر این کربن 5600 سال است. بنابراین پس از این مدت جسم مرده دقیقا نصف تشعشع رادیواکتیو زمان زندگی خود را ساطع می کند. این فروپاشی با آهنگ ثابتی انجام می شود و در نتیجه این امکان وجود دارد که با اندازه گیری میزان تابش زمان مرگ موجود مورد نظر را دریافت. باستانشناسان از عمر بعضی کربن برای یافتن تاریخ مومیایی های مصر باستان استفاده کرده اند.
از دیدگاه نظری ، همه مواد رادیواکتیو نهایتا به سرب تبدیل می شوند ، هسته اتم سرب پایدار است و بنابراین خاصیت رادیواکتیو ندارد.اما این امر به طور تجربی اثبات نشده است. زیرا نیمه عمر بعضی از عناصر بیش از عمر انسانهاست.

سرب پایدار(بدون نیمه عمر)

کاربردها:

بسیاری از ایزوتوپها رادیواکتیو هستند یعنی ذرات با فرکانس بالا را از هسته (مرکز) اتمهای خود ساطع می کنند. از آنها می توان برای دنبال کردن مسیر مواد متحرکی که از دید پنهان هستند ، مانند جریان خون در بدن یک بیمار در بیمارستان ، استفاده کرد.
  • در جریان خون:
مقدار کمی از یک ایزوتوپ رادیواکتیو به درون جریان خون بیمار تزریق می شود. سپس مسیر آن توسط آشکار سازهای خاصی که فعالیت رادیواکتیویته را مشخص می کنند دنبال می شود. این اطلاعات به یک کامپیوتر داده می شود که صفحه آن هرگونه اختلالی مانند انعقاد خون در رگها را نشان می دهد. با استفاده از روشی مشابه ، می توان از ایزوتوپها برای مطالعه جریان مایعات در تاسیسات شیمیایی نیز استفاده کرد.
  • در فرسودگی ماشین آلات:
آهنگ فرسودگی ماشین آلات صنعتی را نیز می توان با استفاده از ایزوتوپها اندازه گرفت. مقادیر اندکی از ایزوتوپها رادیواکتیو به بخشهای فلزی ماشین آلات ، مانند یاتاقانها و رینگ پیسونها اضافه می شود. سپس سرعت فرسودگی با اندازه گرفتن رادیواکتیویته روغنی که برای روغنکاری این بخشها به کار رفته است محاسبه می شود.

اندازه گیری رادیو اکتیویته


خروجی یا تابش رادیواکتیو می تواند وارد بافتهای زنده شود و به آنها صدمه بند ، بنابراین اطراف آن باید کنترل شود . این تابش را با وسیله ای به نام شمارنده گایگر ـمولر ، که نام آن از مخترعانش اقتباس شده است ، می توان اندازه گرفت وقتی تابش رادیو اکتیو وارد این شمارنده می شود ، گاز موجود در آن حامل الکتریسیته می شود . مقدار بار را می توان روی صفحه ای قرائت کرد ، یا از طریق یک بلندگو به صورت صداهای تیک تیک خاصی شنید.

‌فصل اول - كليات
‌ماده 1 - اهداف
‌با توجه به گسترش روزافزون كاربرد اشعه (‌پرتوها) در امور مختلف و ضرورت حفاظت كاركنان، مردم، نسلهاي آينده و محيط در برابر اثرات زيان‌آور‌اشعه، مقررات ذيل تدوين گرديده است:
‌ماده 2 - تعاريف
1 - "‌اشعه" يا "‌پرتوها" شامل اشعه يون‌ساز و غير يون‌ساز مي‌باشد.
2 -"‌منابع مولد اشعه" به مواد پرتوزا (‌راديواكتيو) اعم از طبيعي و يا مصنوعي يا مواد و اشياء حاوي آن و يا دستگاهها و تأسيسات مولد اشعه‌اطلاق مي‌گردد.
3 - "‌كار با اشعه" هر گونه كار يا فعاليتي است كه در ارتباط با منابع مولد اشعه انجام شود.
4 - "‌واحد قانوني" در مفهوم "‌سازمان انرژي اتمي ايران" است.
5 - "‌شخص مسئول" شخص حقيقي است كه برابر آيين‌نامه مربوطه واجد صلاحيت علمي و فني و شرايط لازم براي تصدي و نظارت بر كليه امور‌مربوطه به كار با اشعه در محدوده پروانه مربوطه باشد.
6 - "‌مسئول فيزيك بهداشت" شخص حقيقي است كه برابر آيين‌نامه مربوطه واجد صلاحيت علمي و فني و شرايط لازم براي تصدي مسئوليت‌حفاظت در برابر اشعه در محدوده پروانه مربوطه باشد.
‌ماده 3 - شمول مقررات
‌مقررات اين قانون شامل كليه امور مربوط به حفاظت در برابر اشعه در سطح كشور از جمله موارد زير مي‌باشد:
1 - منابع مولد اشعه
2 - كار با اشعه
3 - احداث، تأسيس، راه‌اندازي، بهره‌برداري، ازكاراندازي و تصدي هر واحدي كه در آن كار با اشعه انجام شود.
4 - هر گونه فعليت در رابطه با منابع مولد اشعه شامل واردات و صادرات، ترخيص، توزيع، تهيه، توليد، ساخت، تملك، تحصيل، اكتشاف،‌استخراج ، حمل و نقل، معاملات، پيمانكاري، نقل و انتقال، كاربرد و يا پس‌مانداري.
5 - حفاظت كاركنان، مردم و نسل‌هاي آينده به طور كلي و محيط در برابر اثرات زيان‌آور اشعه.
‌فصل دوم - پروانه و مسئوليت‌ها
‌ماده 4 - انجام هر گونه فعاليت در ارتباط با موارد مندرج در بندهاي 1، 2، 3، و 4 از ماده 3، غير از موارد مستثني به موجب آيين‌نامه‌هاي مربوطه‌مستلزم اخذ پروانه كسب از واحد ذيربط و پروانه اشتغال از واحد قانوني مي‌باشد.
‌تبصره - مجوز كار با اشعه در مورد مؤسسات پزشكي صرفاً براي متخصصين گروه پزشكي توسط كميسيوني مركب از دو نفر متخصص امور‌حفاظت در برابر اشعه از واحد قانوني و دو نفر كارشناس از وزارت بهداشت، درمان و آموزش پزشكي مورد بررسي و تأييد قرار گرفته و از طرف واحد‌قانوني داده خواهد شد. صدور پروانه نهايي تأسيس واحد كار با اشعه از سوي وزارت بهداشت، درمان و آموزش پزشكي خواهد بود.
‌ماده 5 - دارنده پروانه كسب مكلف است حداقل يك فرد واجد شرايط به عنوان شخص مسئول و يك فرد واجد شرايط به عنوان مسئول فيزيك‌بهداشت به واحد قانوني معرفي تا پروانه اشتغال بنام آنان اخذ گردد.
‌تبصره - در موارد خاص (‌مانند واحدهاي تشخيص با اشعه ايكس با فعاليت محدود) و طبق آيين‌نامه‌هاي مربوط، مسئوليت شخص حقيقي دارنده‌پروانه كسب، شخص مسئول و مسئول فيزيك بهداشت مي‌تواند توأماً به عهده يك يا دو شخص حقيقي واجد شرايط باشد.
‌ماده 6 - دارنده پروانه اشتغال مكلف است منحصراً در محل و در حدود و شرايط مندرج در پروانه و دستورالعملهاي مربوطه فعليت نمايد.
‌ماده 7 - هر گونه تغيير در وضعيت حقوقي دارنده پروانه كسب در ارتباط با منابع مولد اشعه و نيز هر گونه تغيير كمي و كيفي در ارتباط با منابع مزبور‌مستلزم اخذ مجوز از واحد قانوني است.
‌تبصره - در خصوص مؤسسات پزشكي پس از اخذ مجوز از واحد قانوني، صدور پروانه كار جديد از سوي وزارت بهداشت، درمان و آموزش‌پزشكي خواهد بود.
‌ماده 8 - دارندگان پروانه و يا قائم‌مقام قانوني آنها و نيز كليه افرادي كه به لحاظ وظيفه شغلي با منابع مولد اشعه در ارتباط مي‌باشند مكلفند موارد زير‌را بلافاصله به واحد قانوني اطلاع دهند:
1 - تعليق و يا تعطيل بهره‌برداري از منابع مولد اشعه.
2 - مفقود شدن و يا سرقت منابع مولد اشعه.
3 - هر گونه حادثه، اخلال، عيب و يا تغييرات در رابطه با منابع مولد اشعه كه احتمال افزايش مخاطرات بالقوه پرتوگيري افراد را در بر داشته باشد.
4 - سوانح پرتوگيري و نيز پرتوگيري مشكوك افراد.
‌ماده 9 - كليه افرادي كه به كار با اشعه گمارده مي‌شوند بايد تحت معاينات و آزمايشهاي پزشكي لازم قبل و بعد از استخدام و به صورت دوره‌اي‌طبق آيين‌نامه مربوطه قرار گرفته و مدارك لازم را در اختيار واحد قانوني قرار دهند.
‌ماده 10 - گماردن افراد زير به كار با اشعه ممنوع است:
1 - افراد كمتر از 18 سال سن غير از موارد مستثني به موجب آيين‌نامه مربوطه.
2 - افرادي كه در نتيجه آزمايش‌هاي پزشكي مورد تأييد واحد قانوني كار با اشعه براي سلامتي آنان زيان‌آور تشخيص داده شده باشد.
‌ماده 11 - در انجام هر گونه فعاليت مربوط به موارد مندرج در ماده 3 دارنده پروانه كسب، شخص مسئول و مسئول فيزيك بهداشت مكلفند:
1 - كليه مقررات، استانداردها، آيين‌نامه‌ها و دستورالعملهاي حفاظت در برابر اشعه را رعايت نمايند.
2 - كليه تدابير و تجهيزات حفاظتي لازم را طبق مقررات مربوط پيش‌بيني ، تأمين و به اجراء در آورند.
3 - از پرتودهي غير ضروري اجتناب نمايند.
‌ماده 12 - كليه افرادي كه به كار با اشعه اشتغال دارند مكلفند وسائل حفاظت در برابر اشعه پيش‌بيني شده را شخصاً بكار گرفته و مقررات و‌دستورالعملهاي مربوط به اجرا درآورند.
‌فصل سوم - نظارت و بازرسي
‌ماده 13 - واحد قانوني در جهت حسن اجراي مقررات اين قانون، نظارت بر كليه امور مندرج در ماده 3 اين قانون و بازرسي در زمينه‌هاي مزبور را به‌عهده دارد.
‌ماده 14 - دارنده پروانه كسب، شخص مسئول و مسئول فيزيك بهداشت مكلفند توصيه و دستورالعملهاي ابلاغ شده توسط واحد قانوني و‌بازرسين مربوطه را به اجراء درآورند.
‌ماده 15 - در مواردي كه اجراي امور مربوط به مفاد ماده 14 و يا حفاظت افراد و اموال در برابر اشعه مستلزم ارائه خدمات از طرف واحد قانوني‌باشد، اشخاص ذينفع مكلفند بهاء خدمات ارائه شده را طبق تعرفه مقرر در آيين‌نامه مربوطه به حساب خزانه‌داري كل واريز نمايند.
‌تبصره - در مورد مؤسسات پزشكي كه منجر به صرف هزينه يا ارائه خدمات توسط وزارت بهداشت، درمان و آموزش پزشكي مي‌شود آيين‌نامه‌مربوطه توسط وزارت بهداشت، درمان و آموزش پزشكي تهيه مي‌گردد.
‌ماده 16 - دارنده پروانه و يا هر شخصي كه به نحوي از انحاء منابع مولد اشعه را برابر مفاد اين قانون در اختيار و يا تحت نظر داشته و يا به عنوان‌شخص مسئول و يا مسئول فيزيك بهداشت انجام وظيفه نمايد مكلف است در حوزه فعاليت شغلي خود تسهيلات لازم براي اعمال نظارت و بازرسي‌واحد قانوني را فراهم نموده و اطلاعات و مدارك مورد نياز را در اختيار واحد قانوني قرار دهد.
‌ماده 17 - واحد قانوني مكلف است در اجراي مقررات اين قانون، در صورت وقوف بر وجود اشكالات يا تخلفاتي در كار با اشعه يا بهره‌برداري از‌منابع مولد اشعه پس از ابلاغ كتبي مدت‌دار به وزارت و يا مؤسسه مربوطه و در صورت عدم رعايت توصيه‌ها، دستور توقف و يا تعطيل بهره‌برداري از‌منابع مربوطه را صادر نموده و يا پروانه صادره را لغو نمايد و در صورت لزوم با اخذ مجوز لازم از مرجع ذيصلاح اقدام به لاك و مهر آن بنمايد.
‌فصل چهارم - جرائم و مجازات‌ها
‌ماده 18 - موارد زير جرم محسوب و مرتكب حسب مورد با رعايت شرايط و امكانات خاطي و دفعات و مراتب تأديب از وعظ و توبيخ و تهديد و‌درجات تعزير به مجازات مشروحه ذيل محكوم خواهد شد:
1 - عدم استفاده از وسايل حفاظتي پيش‌بيني شده و همچنين عدم رعايت دستورالعملهاي حفاظتي توسط كاركنان با اشعه جرم محسوب و‌متخلف به جريمه نقدي از ده هزار ريال تا صد و پنجاه هزار ريال محكوم خواهد شد.
2 - گماردن افراد بدون انجام آزمايشهاي پزشكي لازم قبل از استخدام و يا بدون مراقبتها و آزمايشهاي دوره‌اي پزشكي در مدت اشتغال بكار با‌اشعه جرم محسوب و متخلف به جريمه نقدي از ده هزار ريال تا پانصد هزار ريال محكوم خواهد شد.
3 - گماردن افرادي بكار با اشعه كه به موجب ماده 10 اين قانون كار با اشعه براي آنان ممنوع اعلام شده است جرم محسوب و متخلف به جريمه‌نقدي از سي هزار ريال تا يك ميليون ريال محكوم خواهد شد.
4 - كوتاهي در اعلام موارد مذكور در مواد 7 و 8 و 9 اين قانون توسط اشخاص نامبرده در موارد ياد شده جرم محسوب و متخلف به جريمه نقدي‌از سي هزار ريال تا دو ميليون ريال محكوم خواهد شد.
5 - متخلف موارد زير به جزاي نقدي از سي هزار ريال تا پنج ميليون ريال و يا به حبس تعزيري از يك ماه تا شش ماه و يا به هر دو مجازات‌محكوم خواهد شد:
‌الف - بهره‌برداري از منابع مولد اشعه و يا كار با اشعه بدون اتخاذ تدابير حفاظتي و تدارك تجهيزات حفاظتي توصيه شده توسط واحد قانوني.
ب - بهره‌برداري از منابع مولد اشعه و يا كار با اشعه بدون نظارت شخص مسئول و مسئول فيزيك بهداشت.
ج - اخلال در امر نظارت و بازرسي واحد قانوني و ندادن اطلاعات لازم و يا ارائه اطلاعات ناقص و يا كذب به واحد قانوني و نيز هر اقدامي كه‌موجب انحراف تشخيص واحد قانوني گردد.
6 - ايجاد اختلال در كار با اشعه و يا منابع مولد اشعه جرم محسوب و متخلف به جزاي نقدي از پانصد هزار تا پانزده ميليون ريال و حبس تعزيري‌از يك ماه تا سه سال و يا به هر دو مجازات محكوم خواهد شد.
7 - عدم رعايت حدود و شرايط مقرر در پروانه يا تغيير در شرايط مزبور جرم محسوب و متخلف به مجازات نقدي از يك ميليون تا پنج ميليون‌ريال و يا به حبس تعزيري از شش ماه تا دو سال و يا به هر دو مجازات محكوم خواهد شد.
8 - نداشتن پروانه معتبر در مواردي كه به موجب ماده 4 اين قانون داشتن پروانه الزامي اعلام گرديده است جرم محسوب و متخلف به مجازات‌نقدي از يك ميليون ريال تا ده ميليون ريال و يا به حبس تعزيري از شش ماه تا سه سال و يا به هر دو مجازات محكوم خواهد شد.
9 - بهره‌برداري از منابع مولد اشعه كه توسط واحد قانوني به نحوي ممنوع اعلام شده است جرم محسوب مي‌شود و متخلف به مجازات نقدي از‌يك ميليون ريال تا پانزده ميليون ريال و يا به حبس تعزيري از شش ماه تا سه سال و يا به هر دو مجازات محكوم خواهد شد.
‌ماده 19 - در مواردي كه جرائم موضوع اين قانون به لحاظ انطباق با عناوين قانوني ديگر مستلزم مجازات شديدتر باشد مقررات قانون مجازات اشد‌در باره مرتكب اعمال خواهد شد.
‌فصل پنجم - مقررات ويژه
‌ماده 20 - به افرادي كه به طور مستمر به كار با اشعه اشتغال داشته باشند، مزاياي زير بر مبناي مقدار و شرايط بالقوه پرتودهي محيط كار به تشخيص‌واحد قانوني و طبق آيين‌نامه‌هاي مربوط تعلق مي‌گيرد.
1 - كاهش ساعات كار هفتگي تا ميزان 25% ساعات كار مقرر براي ساير كاركنان.
2 - افزايش ميزان مرخصي استحقاقي ساليانه تا يك ماه در سال براي مدت اشتغال به كار با اشعه، استفاده از مرخصي استحقاقي ساليانه در آن گونه‌موارد در طول هر سال اجباري است.
3 - افزايش مدت خدمت مورد قبول تا يك سال به ازاء هر يك سال كار با اشعه، حداكثر اين افزايش تا ده سال و منحصراً از نظر بازخريد،‌بازنشستگي، ازكارافتادگي و تعيين حقوق وظيفه قابل احتساب مي‌باشد.
4 - پرداخت تا پنجاه درصد حقوق و مزايا به عنوان فوق‌العاده كار با اشعه.
‌تبصره - در مورد بند 2 اين ماده به جاي استفاده از مرخصي فرد ذينفع مي‌تواند درخواست اشتغال در محلي غير از محيط كار با اشعه بنمايد.
‌ماده 21 - وزارتخانه‌ها، نهادهاي انقلاب اسلامي، مؤسسات، سازمانها و شركتهاي دولتي و يا وابسته به دولت و مؤسساتي كه شمول قانون بر آنها‌مستلزم ذكر نام است و نيز كليه مأمورين انتظامي موظفند در اجراي اين قانون با واحد قانوني همكاري نمايند.
‌ماده 22 - واحد قانوني مسئوليت حسن اجراي مقررات اين قانون را به عهده داشته و مكلف است با بكار گماردن متخصصين واجد صلاحيت‌علمي و فني و از طريق تهيه و تدوين ضوابط، مقررات، استانداردها و دستورالعملهاي لازم و به كارگيري امكانات تخصصي، آموزش و پژوهش و ارائه‌خدمات در سطح علمي پيشرفته روز تدابير مقتضي را اتخاذ نمايد.
‌ماده 23 - اين قانون از تاريخ تصويب لازم‌الاجرا مي‌باشد و از تاريخ مزبور كليه قوانين و مقررات مغاير لغو و كان‌لم‌يكن تلقي مي‌گردد. دولت مكلف‌است آيين‌نامه‌هاي مربوطه را بر اساس پيشنهاد واحد قانوني تصويب و جهت اجراء ابلاغ نمايد. كليه اشخاص حقيقي و حقوقي مشمول اين قانون‌مكلفند حداكثر ظرف شش ماه از تاريخ اجراي قانون وضعيت خود را با مقررات آن منطبق نمايند.
‌تبصره - در خصوص مؤسسات پزشكي كشور آيين‌نامه‌هاي مربوطه توسط وزارت بهداشت، درمان و آموزش پزشكي و واحد قانوني تهيه و تدوين‌و پس از تصويب هيأت وزيران قابل اجراء خواهد بود.
‌قانون فوق مشتمل بر بيست و سه ماده و شش تبصره در جلسه علني روز يكشنبه مورخ بيستم فروردين ماه يك هزار و سيصد و شصت و هشت‌مجلس شوراي اسلامي تصويب و در تاريخ 1368.1.30 به تأييد شوراي نگهبان رسيده است.

What does “imaging” mean?

In medicine, “imaging” is the general term for any technique used to provide pictures of bones and organs inside the body. Imaging techniques consist of x rays, ultrasound, magnetic resonance imaging (MRI), and computerized tomography (CT) scans. Imaging helps doctors see the causes of medical problems.

What is the urinary tract?

The urinary tract consists of the organs, tubes, and muscles that work together to make, move, store, and release urine, the liquid waste of the human body. The upper urinary tract includes the kidneys, which filter wastes and extra fluid from the blood, and the ureters, which carry urine from the kidneys to the bladder. The lower urinary tract includes the bladder, a balloon-shaped muscle that stores urine, and the urethra, a tube that carries urine from the bladder to the outside of the body during urination. Doctors who specialize in kidney problems are called nephrologists. Doctors who specialize in problems of the organs and tubes that transport urine from the kidneys to outside the body are called urologists. These problems may involve cancers or growths of these organs, including the kidneys, ureters, bladder, and testes, or may involve abnormalities in storing or releasing urine.

What problems could require imaging of the urinary tract?

Imaging may help your doctor find the cause of

  • urinary incontinence—unintended leakage of urine
  • frequent, urgent urination
  • blockage of urine
  • abdominal mass
  • pain in the groin or lower back
  • blood in the urine
  • high blood pressure
  • kidney failure

One symptom could have several possible causes. Your doctor can use imaging techniques to determine, for example, whether a urinary stone or an enlarged prostate is blocking urine flow. Imaging can also help clarify kidney diseases, tumors, urinary reflux—backward flow of urine—urinary tract infections, incomplete emptying, and small bladder capacity.

What factors will my doctor consider before ordering tests?

The first step in solving a urinary problem is to talk with your doctor. You will be asked about your general medical history, including any major illnesses or surgeries, so you should be prepared to give as many details as you can about the problem and when it started. You should mention all the medicines you take, both prescription and nonprescription, because they might be part of the problem. You should also talk about how much fluid you drink a day and whether the beverages you drink contain alcohol or caffeine.

 

Why does the doctor choose one imaging technique instead of another?

Your doctor will look at several factors to decide what imaging technique to use. Each has advantages and disadvantages. Convenience and cost-effectiveness are also factors.

Conventional radiology. Doctors have used x-ray machines to diagnose diseases for about a century. X rays of the urinary tract can help highlight a kidney stone or tumor that could block the flow of urine and cause pain. An x ray can also show the size and shape of the prostate. Conventional x rays do involve some exposure to ionizing radiation—radiation that is strong enough to damage some cells. Two x-ray procedures involve the use of contrast medium, which is a liquid that acts like a dye and shows the shape of the urinary tract as it passes through the tract.

Intravenous pyelogram (IVP).

  • Intravenous pyelogram (IVP)—In an IVP, the contrast medium is injected into a vein, usually in the arm. The radiologist takes a series of snapshots as the medium circulates through the blood and reaches the kidneys. The structure of the kidneys shows up clearly on the x rays as the contrast medium is filtered from the blood and passes through the kidneys to the ureters.

  • Voiding cystourethrogram (VCUG)—In a VCUG, a catheter inserted in the urethra is used to fill the bladder with the contrast medium. The x-ray machine then captures a video of the contrast medium during urination. This procedure allows the doctor to see things such as whether urine is backing up into the ureters when it should be traveling the other way, down through the urethra, or whether urine outflow through the urethra is blocked. VCUG is often used with children who have recurrent infections to determine whether a defect in the urinary tract is causing the infections. It can also show blockages from an enlarged prostate in men or abnormal bladder position in women.

Ultrasound. In ultrasound, or sonography, a technician holds a device, called a transducer, that sends harmless sound waves into the body and catches them as they bounce back off the internal organs to create a picture on a monitor. Different angles make it possible to examine different organs.

  • Abdominal ultrasound—In abdominal ultrasound, the technician applies a gel to the patient’s abdomen and holds the transducer against the skin. The gel allows the transducer to glide easily, and it improves the transmission of the signals. Abdominal ultrasounds are well known for taking pictures of fetuses in the womb and of a woman’s ovaries and uterus, but this approach can also be used to evaluate the size and shape of the kidneys.

  • Transrectal ultrasound—Transrectal ultrasound is most often used to examine the prostate. The transducer is inserted into the patient’s rectum so that it is right next to the prostate. The ultrasound image shows the size and shape of the prostate and any irregularity that might be a tumor. To determine whether an abnormal-looking area is in fact a tumor, the doctor can use the transducer and the ultrasound images to guide a biopsy needle to the suspected tumor. The needle collects a few pieces of prostate tissue for examination with a microscope.

MRI. MRI machines use radio waves and magnets to produce detailed pictures of internal organs and tissues. No exposure to radiation occurs. With most MRI machines, the patient lies on a table that slides into a tunnel that may be open-ended or closed at one end. Some newer machines are designed to allow the patient to lie in a more open space. During an MRI, the patient is awake but must remain perfectly still while the images are being taken, usually only a few minutes. A sequence of images may be needed to create a detailed picture of the organ. During the sequencing, the patient will hear mechanical knocking and humming noises.

Magnetic resonance angiogram (MRA).

  • Magnetic resonance angiogram (MRA)—An MRA provides the most detailed view of renal arteries. It can show renal artery stenosis, which is a narrowing of vessels that causes poor blood flow to the kidney and can cause high blood pressure and lead to reduced kidney function and eventually to kidney failure.

CT scan. CT scans use a combination of x rays and computer technology to create three-dimensional images. Like MRIs, CT scans require the patient to lie on a table that slides into a tunnel. CT scans can help identify stones in the urinary tract, infections, cysts, tumors, and traumatic injury to the kidneys and ureters.

 

How do I prepare for an imaging examination?

How you prepare will depend on the purpose of the examination and the type of equipment to be used. You should listen to your doctor’s instructions carefully and ask questions if you do not understand something that is said.

  • Your doctor needs to know if you have any allergies to foods or medications and if you have had any recent illnesses or medical conditions.

  • Your doctor may tell you not to eat or drink anything for 12 hours before the test. For some ultrasound examinations, however, you may be instructed to drink several glasses of water 2 hours before the examination so your bladder will be full.

  • You may be given a laxative to clear the colon before the examination. If you are having a transrectal ultrasound, you will be given an enema about 4 hours before the examination.

  • If you are having an MRI or MRA, talk with the technical staff about any implanted devices—such as heart pacemakers, intrauterine devices (IUDs), hip replacements, and implanted ports for catheterization—that may have metal parts that will affect the images. Metal plates, pins, screws, and surgical staples, as well as any bullets or shrapnel you might have in your body, may cause a problem if they have been in place for less than 4 to 6 weeks.

  • If you feel uneasy in enclosed spaces, you may need to have a sedative before an MRI or a CT scan.

 

What are the test procedures like?

Most procedures for imaging the urinary tract are performed as the patient lies on a table.

  • For an IVP, dye is injected into a vein, and x-ray pictures are taken at 0, 5, 10, and 15 minutes to see the progression of the contrast medium through the kidneys and ureters. The dye makes the kidneys and urine visible on the x ray and shows any narrowing or blockage in the urinary tract. This procedure can help identify problems in the kidneys, ureters, or bladder that may have resulted from urine retention or backup.

  • MRI and CT scans may also require injection of dye. You will be asked to lie still for minutes at a time as the equipment takes pictures from different angles. A computer puts the different views together to create a three-dimensional model of your urinary tract. Some patients find it uncomfortable to lie in the small imaging tunnel, and others find the equipment noises annoying or unsettling. Knowing what to expect helps make these aspects of the test less disturbing.

  • VCUG is most often used to evaluate urinary problems in children. The doctor or nurse cleans the area around the urethra, inserts the tip of the catheter, and gently slides it up into the bladder. The child’s bladder is first emptied. Contrast medium is slowly dripped into the bladder, by means of gravity, until the bladder is full. X-ray films are then made as the child urinates.

  • For a transrectal ultrasound, the doctor or technician inserts a probe slightly larger than a pen into the rectum. The probe directs high-frequency sound waves at the prostate and the echo patterns form an image of the gland on a monitor. Although the image shows the size of the prostate and any irregularities, it cannot definitively identify tumors. To determine whether an abnormal-looking area is in fact a tumor, the doctor can use the probe and the ultrasound images to guide a biopsy needle to the area. The needle collects a few pieces of prostate tissue for examination with a microscope.

  • For an abdominal ultrasound exam, a technician will apply gel to your abdomen and sweep a handheld transducer across the area to generate a picture of your urinary tract. Like the IVP, an abdominal ultrasound can show damage or abnormalities in the upper urinary tract.

 

What should I do after the test?

For most of these tests, you will be able to resume normal activity immediately afterward. If your test involved placing a catheter in the urethra, you may have some mild discomfort. Drinking an 8-ounce glass of water every 30 minutes for 2 hours should help. Also, you may be able to take a warm bath. Alternatively, holding a warm, damp washcloth over the urethral opening may relieve the discomfort.

You may experience some discomfort after a transrectal ultrasound as well. A prostate biopsy may produce pain in the rectum and the perineum—the area between the rectum and the scrotum.

For catheterization or biopsy, your doctor will sometimes, but not always, give you an antibiotic to take for 1 or 2 days to prevent an infection. If you notice signs of infection—including chills, fever, or persistent pain when you urinate—you should call your doctor at once.

 

When will I get the results?

Results for simple tests can be discussed with your doctor or nurse immediately after the test. Other results may take a few days. You will have the chance to ask questions about the results and possible treatments for your problem.

 

Hope through Research

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) has many research programs aimed at understanding kidney and urologic disorders, including urinary stones, enlarged prostate, urinary incontinence, and kidney failure. The NIDDK has established a program to develop and test accurate, reproducible techniques to monitor the progression of polycystic kidney disease so that potential interventions can be evaluated. This program will apply the latest advances in imaging technology so that clinicians can use information about kidney size and the portion of the kidney occupied by cysts to determine how far the disease has progressed. For example, current state-of-the-art methods using MRI techniques with rapid image acquisition rates make possible high-resolution, three-dimensional images of the kidneys. Semiautomated image analysis can determine renal size and the location of cystic structures. MRI may also permit simultaneous estimation of kidney function.

Reducing Radiation Risk from Computed Tomography for Pediatric and Small Adult Patients

While the benefits of computed tomography are well known in diagnosing diseases and trauma and in the guidance of interventional and therapeutic procedures, those benefits are not without risks.  This Notification is to emphasize the importance of keeping radiation doses during CT procedures as low as reasonably achievable, especially for pediatric and small adult patients, who may sometimes receive more radiation than needed to obtain diagnostic images.  To prevent this, we want to stress the importance of adjusting CT scanner parameters appropriately for each individual’s weight and size, and for the anatomic region being scanned.

Background

The individual risk from the radiation associated with a CT scan is quite small compared to the benefits that accurate diagnosis and treatment can provide.  Still, unnecessary radiation exposure during medical procedures should be avoided.  This is particularly important when the patient is a child, since children exposed to radiation are at a relatively greater risk than adults. 1  The American College of Radiology has noted, “Because they have more rapidly dividing cells than adults and have longer life expectancy, the odds that children will develop cancers from x-ray radiation may be significantly higher than adults.”2  It has been estimated by the National Research Council's Committee on the Biological Effects of Ionizing Radiation that children less than 10 years of age are several times more sensitive to radiation than middle-aged adults.3  Unnecessary radiation may be delivered when CT scanner parameters are not appropriately adjusted for patient size.4  When a CT scan is performed on a child or small adult with the same technique factors that are used for a typically-sized adult, the small patient receives a significantly larger effective dose than the full-sized patient.

To compound the problem, the overexposure of children or small adults during CT procedures can easily go unrecognized.  In conventional x-ray procedures, medical personnel can tell if the patient has been overexposed because the resulting film is overexposed, producing a dark image.5  But with CT, there is no obvious evidence that the patient has been overexposed because the quality of the image may not be compromised.  

Several recent articles stress that it is important to use the lowest radiation dose necessary to provide an image from which an accurate diagnosis can be made, and that significant dose reductions can be achieved without compromising clinical efficacy.2, 5, 6 , 7, 8, 9, 10  

Recommendations

Here are the steps we are recommending.  They are not new.  Indeed, many facilities are already taking measures to protect children and other small patients from unnecessary exposure during CT procedures.11, 12, 13

  1. Optimize CT Settings.  Based on patient weight or diameter and anatomic region of interest, evaluate whether your CT operating conditions are optimally balanced between image quality and radiation exposure.  To reduce dose while maintaining diagnostic image quality:

    • Reduce tube current.  With all other factors held constant, patient radiation dose is directly proportional to x-ray tube current.  For example, a 50 percent reduction in tube current results in a 50 percent decrease in radiation dose.9
    • Develop and use a chart or table of tube-current settings based on patient weight or diameter and anatomical region of interest. See reference 9 for an example of tube current settings based on patient weight and anatomical region of interest (i.e., chest, pelvis or abdomen) for a single-detector helical-scanning CT unit.  The diameter of the patient may be a better predictor of the tube-current required than body weight because patient diameter better correlates with the x-ray beam attenuation in the patient.10  Your facility’s medical physicist and the scanner manufacturer can help in developing this chart or table.
    • Increase table increment (axial scanning) or pitch (helical scanning).   If the pitch is increased, the amount of radiation needed to cover the anatomical area of interest is decreased.2, 14 One study showed that increasing the pitch from 1:1 to 1.5:1 decreased the radiation dose by 33 percent without loss of diagnostic information.15  Consult your facility’s medical physicist, who can advise you on optimal tube-current and pitch settings for diagnostic requirements.  You can also contact the manufacturer of the CT scanner for recommendations specific to your model.

      Note that  some newer CT scanners may automatically suggest or implement an increase in mA if pitch is increased.  For these models,  increasing the pitch may not result in a lower radiation dose.  Contact the CT scanner’s manufacturer for recommendations on your model’s automatic current adjustment features.

  1. Reduce the number of multiple scans with contrast material.  Often, CT scans are done before, during, and after injection of IV contrast material.  When medically appropriate, multiple exposures may be reduced by eliminating pre-contrast images (i.e., unenhanced images). 9
  2. Eliminate inappropriate referrals for CT.  In some cases,  conventional radiography, sonography, or magnetic resonance imaging (MRI) can be just as effective as CT, and with lower radiation exposure.  Most conventional x-ray units deliver less ionizing radiation than CT systems, and sonography and MRI systems deliver no x-ray radiation at all.  It is important to triage these examinations to eliminate inappropriate referrals or to utilize procedures with less or no ionizing radiation.9
